- Department of Radiology and Imaging Sciences Statement of Work for Service Agreement on Hologic Selenia Digital Mammography System (POTS #11002924) BACKGROUND: The Department of Radiology and Imaging Sciences provides imaging services to the National Institutes of Health Clinical Center’s patients who participate in research protocols conducted by the various NIH institutes. An imaging service essential to patient care includes the Hologic Selenia Digital Mammography Unit (S/N 28403083025) and the associated R2 CAD Image Checker (S/N D2114). Mammography is a highly regulated imaging modality with mandatory accreditation by the American College of Radiology (ACR) and compliance with the Food and Drug Administration’s (FDA) Mammography Quality Standards Act (MQSA). PURPOSE: A service agreement is required to ensure continuous operation (in compliance with ACR and MQSA) of the above listed mammography system and minimal down-time for repairs and/or preventative maintenance (PM). CONTRACTOR REQUIREMENTS: The following services shall be provided for the Mammography Unit and R2 CAD Image Checker system referenced above: a. Telephone diagnostic and repair support 24 hours/day, 365 days/year. b.Onsite repair services including all labor and travel with a coverage period of 8 a.m. to 9 p.m. Monday through Friday, excluding holidays. b. Replacement of parts with identical (or upgraded) original equipment manufacturer (OEM) parts; including glassware, x-ray tube and digital array detector. All replacement parts shall be OEM; in particular, x-ray tube replacement (part number 2-425-0037), digital array detector (part number RM-ASY-01389), and computer (part number RM-CMP-00356). c.Provide OEM commercially available software updates with installation during normal business hours Monday through Friday, 8 a.m. – 5 p.m.; d.Perform two (2) Preventative Maintenance (PM) calls per year following the OEM’s preventative maintenance protocol in order to maintain the equipment in an operating condition consistent with the specifications of the original equipment manufacturer (OEM) and in accordance with the current FDA MQSA regulations during normal business hours Monday through Friday, 8 a.m. – 5 p.m.; e.Replacement coverage for all paddle plastics and face shields broken during coverage period. Provide five spare paddles shipped at the beginning of the service agreement period of performance. Specific paddles to be chosen by NIH prior to shipment. f.Equipment must be maintained and repaired in compliance with current MQSA and ACR requirements. g.Provide telephone consultation and when necessary on-site consultation when medical physicist’s MQSA testing demonstrates non-compliance with MQSA regulations. GOVERNMENT RESPONSIBILITIES: Provide a safe work environment and access for the service/preventative maintenance on the equipment. REPORTING REQUIREMENTS AND DELIVERABLES: Contractor shall provide a field service report to the Project Officer describing diagnosed problems and solutions upon completion of the work. PROGRAM MANAGEMENT AND CONTROL REQUIREMENTS: N/A INSPECTION AND ACCEPTANCE REQUIREMENTS: Verification of the repair will be performed on the equipment by the Senior Medical Physicist (or his designee) upon completion of the repair. PERIOD OF PERFORMANCE: One (1) year, 11JUN11 – 10JUN12 (base year), with four (4) option years to be exercised at the Government’s discretion.
