Solicitation Number: VA-241-11-RQ-0676 Notice Type: Combined Synopsis/Solicitation Synopsis: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA-241-11-RQ-0676 and is issued as an RFQ. (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-35, July 14, 2009. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at https://www.acquisition.gov/far/ (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as a sole source requirement. VISN 1 Contracting Activity intends to negotiate a sole source contract under the authority of FAR Part 6.302-1, only one responsible source to the vendor, Illumina, Inc., who is a large business, and they are located at 9865 Towne Centre Drive, San Diego, CA 92121. However, all other responsible sources that feel they can provide this or a similar item may submit a capability statement, proposal, or quotation, which shall be considered by the agency. The North American Industry Classification System (NAICS) code is 337910, and the size standard is 500 employees. (v) This requirement consists of one (1) line item: Part Number #: WG-311-1124 - HumanOmni1-Quad v1.0 DNA Analysis BeadChip Kit (96 samples, multi-use) Qty: 10 ea. Product Information The four-sample HumanOmni1-Quad BeadChip contains strategically selected content that includes markers derived from the 1,000 Genomes Project, all three HapMap phases, and recently published studies. Each BeadChip features over one million available assays per sample, containing carefully selected markers that deliver dense genome-wide coverage. Intelligent tag SNP selection has been optimized to maintain comprehensive genomic coverage, while reducing tag SNP redundancy. This has enabled the inclusion of additional content carefully chosen to target high-value regions of the genome associated with human disease. Each HumanOmni1-Quadv1.0 DNA Analysis Kit multiuse (96) includes reagents to process a total of 96 DNA samples. This kit provides beadchips and reagents enabling the Infinium MultiUse workflow allowing for a single amplification step and usage on up to two additional Omni arrays. The kits will be used with the Illumina ® BeadArray Reader. System: iScan, BeadStation, BeadArray Reader, HiScanSQ Technology: BeadArray Assay: Infinium DNA Analysis Assay Applications: Cytogenetic Analysis, Whole-Genome Genotyping and Copy Number Analysis Species: Human Contents: Each kit contains 4 to 96 BeadChips, along with reagents for amplifying, fragmenting, hybridizing, labeling, and detecting 16 to 384 whole-genome genotyping samples (vii) Delivery FOB Destination to: VA Boston Healthcare System Jamaica Plain Campus 150 South Huntington Ave. Jamaica Plain, MA 02130 More information will be provided upon contract award.
