Combined Synopsis/Solicitation This is a combined synopsis/solicitation for Brand name (Rees Scientific) commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) and the solicitation number is FA7014-11-T-0021. The North American Industry Classification System code is 334290 with a business size standard of 750 employees. CONTRACT LINE ITEMS: 0001: Qty: 1 Package Purchase and Installation of additional card readers for bldg 116. Programming of System, IQ/OQ & Training. Performance Work Statement 1. DESCRIPTION OF SERVICE. Provide and install proximity card readers, door locking mechanisms, call boxes and alarm systems listed below to an existing security system manufactured by REES Scientific Corporation, located at Air Force Mortuary Affairs Operations, 116 Purple Heart Drive, Dover AFB, DE 19902. NOTE: The successful vendor and all employees who would have any interaction with the facility would be required to sign a non-disclosure agreement which prohibits the signatory from discussing anything they see, hear or smell while in the facility. Air Force Mortuary Affairs will take every action possible to protect the dignity of the honored dead, and the sensitivity of contractor personnel; however, employees must be made aware of the fact that they could be subjected to sights, noises or smells that could be very disturbing. 2. SCOPE: Contractor shall supply and install all equipment and materials required to perform the installation of additional card-access type equipment compatible with the current system. All of the equipment and materials supplied shall be of similar make, model and manufacture as the existing equipment used throughout the facility and compatible with the existing CENTRON Access Control System and existing ID card technologies. The contractor shall fully test and verify the functionality of newly installed equipment with the government and provide a minimum one year warranty on all parts and labor. 3. SPECIFICATIONS: 3.1. Vendor shall install and/or relocate access control components as follows: 3.1.1. Install the necessary controller board(s), battery backups and power supplies in room 112 (Communications room) to accommodate five additional card readers. 3.1.2 Install additional magnetic locks and readers on four refrigeration doors in room 103 (Refrigeration room). 3.1.3 Install egress buttons inside each refrigeration compartment in room 103. 3.1.4 Install a panic button or device with audible and visual alarm on each of the refrigeration compartments in room 103 with an additional alarm indicator located in room 12 (C3 Operations). 3.1.6 Install an additional magnetic lock and reader on the sliding door in room 54 (Maceration room). 3.1.7 Install an egress button inside room 54 (Maceration room). 3.1.8 Relocate the exterior front door proximity reader and call box to the door inside the vestibule. 3.1.9 Relocate the call box from room 13 (C3 Chief's Office) to room 12 (C3 Operations). 4. RECORDS MANAGEMENT. The contractor understands that when creating, handling and maintaining records for the Air Force, either electronic or paper, you must meet the requirements established in AFRIMS Records Disposition Schedule (RDS), AFI 33-322, Records Manage Program, AFI 37-138, Records Disposition Procedures and Responsibilities, and AFM 37-123, Management of Records. In order to meet the requirement established IAW the above mentioned regulations contact the Base Records Management Office (436 CS/SCXD, Bldg 124) at (302) 677-3642 for guidance and training. 5. SECURITY. 5.1. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the governmental installation shall abide by all security instructions and directives of Dover AFB. Employees are responsible for safeguarding all government property provided for contractor use. 5.2. At the close for each work period, government facilities, equipment and materials shall be secured, lights, heat and water turned off and all doors and window secured. 5.3. The contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population, nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified during the Criminal Background Check (CBI). 5.4. Contractors shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. Employees and subcontractors later found to be undocumented or illegal aliens will be remanded to the proper authorities. 5.5. The contractor shall not be entitled to any compensation for delays or expenses associated with complying with the provisions of this clause. Furthermore, nothing in this clause shall excuse the contractor from proceeding with the contract as required. 5.6. All contractors and subcontractors when working in a Controlled, Restricted or other sensitive areas must be escorted at all times. The military agency or unit responsible for the project or work is responsible for providing the escorts. The Contractor shall follow existing procedures and instructions for obtaining entrance to restricted or controlled areas. 6. HOURS OF OPERATION: Bldg 116 will be available for the vendor to perform maintenance operations from Monday through Friday, 0800 - 1700, excluding federal holidays. 7. The following factors shall be used to evaluate offers: (i) technical capability of the item offered to meet the Government requirement; (ii) price; (iii) and past performance (see FAR 15.304); Technical and past performance, when combined, are approximately equal to cost or price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of provision) FAR 52.212-3, "Offeror Representations and Certifications-Commercial Items" applies to this acquisition and offeror(s) must include, a completed copy with its offer. An offeror shall complete only paragraph (j) of this provision if the offeror has completed the annual representations and certificates electronically at ORCA. If an offeror has not completed the annual representations and certifications at the ORCA website, the offeror shall complete only paragraphs (b) through (i) of this provision. To be eligible for an award, all contractors must be registered in the Central Contractor Registration (CCR) database.
