Attachment B-Water Level for each requirement RFQ Template for OWS & Grease Traps Performance Work Statement (PWS) for Oil & Water Separators & Grease Traps (dated 11 May 11). Note a few changes/additions to certain Sections in PWS: FEW CHANGES/ADDITIONS TO THE PERFORMANCE WORK STATEMENT FOR OWS & GREASE TRAPS (DATED 11 MAY 11) AND INCLUDED ATTACHMENT B-WATER LEVEL FOR EACH REQUIREMENT, AS NOTED IN BOLD BELOW: The 21st Contracting Squadron, Peterson Air Force Base (AFB), Colorado announces a combined synopsis/solicitation for a non-personal service to maintenance, inspect, pump, clean, remove and transport the Oil & Water Separators, Sand Sediments and Grease Traps on Peterson Air Force Base (AFB), Colorado in accordance with (IAW) the format in Subpart 12.6 to the Federal Acquisition Regulation (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, FA2517-11-T-5006, is being issued as a Request for Quotation (RFQ) to maintain, inspect, pump, clean, remove and transport the Oil & Water Separators, Sand Sediments and Grease Traps on Peterson Air Force Base (AFB), Colorado and the government contemplates awarding a Firm Fixed Price (FFP) purchase order. The proposed purchase order will be set-aside 100% for small business using the following North American Industrial Classification System (NAICS) codes of either-562111 "Solid Waste Collection" or 562212 "Solid Waste Landfill" or 562219 "Other Nonhazardous Waste Treatment & Disposal" all with a size standard of $12.5 million. Scope: Provide all management, tools, supplies, equipment, transportation, labor and other items necessary to provide maintenance, inspect, pump, clean, remove and transport the Oil & Water Separators, Sand Sediments and Grease Traps on Peterson AFB, Colorado, as described in the attached Performance Work Statement (PWS) dated 11 May 11 and Attachments : Attachment A-Analytical Sampling Results Report via Tetra Tech, Attachment B-Water Level for each requirement and (as noted in the PWS) other Attachments provided at award. Anticipate award date to be: 1 June 2011 for a 12 month period. Performance period: 1 June 2011 through 31 May 2012. Prospective Contractor quotes shall utilize the below line item structure when preparing their quote: Review the attached Performance Work Statement (PWS) and Appendix A-Estimated Workload Data, section 5.1.1 for the Oil/Water Separator and Sediment Traps (adding Building 2028) and Section 5.1.2 for the Grease Traps (deleting Building 2019) listed below: Oil & Water Separators with CLINs 0001 & 0002: Non-personal services for maintenance, inspect, pump, clean, remove and transport the Oil & Water Separators, Sand Sediments on Peterson AFB IAW the PWS, Section 1.3.1.-Routine Maintenance and Section 1.3.2-Emergency Service Calls and Appendix A, Section 5.1.1 and 5.1.3. There will be 2 line items (CLINs) for Scheduled Oil & Water Separator Maintenance and Emergency on-call services. 0001: Routine scheduled maintenance at different frequencies (annually or twice annually) IAW the Performance Work Statement (PWS), Appendix A. 0002: Emergency (On-Call) Service within a 12-month period. Emergency on-call services: Grease Traps with CLINs 0003 & 0004: Non-personal services for maintenance, inspect, pump, clean, remove and transport the Grease Traps on Peterson AFB IAW the PWS, Section 1.3.2.-Routine Maintenance and Section 1.3.2-estimated 5 Unscheduled/ Emergency Service Calls and Appendix A, Section 5.1.2. Because each Grease Trap Building has different gallon capacity, each building will be broken down individually listed below under 0003 line items (CLINs) for the Grease Traps Maintenance and 0004 Unscheduled/Emergency on-call services. 0003: Routine scheduled maintenance of Grease Traps (10 Buildings) IAW Appendix A, Section 5.1.2. 0003AA: Bldg 1-Hartinger Bldg (monthly cleaning) 0003AB: Bldg 2-NORTHCOM Bldg (Second Floor) (monthly cleaning) 0003AC: Bldg 2-NORTHCOM Bldg (Janitor closet) (every 2 months) 0003AD: Bldg 218-Fire Station (semi-annual) 0003AE: Bldg 1013-Officer's Club Bldg (every 2 months) 0003AF: Bldg 1054-Golf Course Club House (Quarterly) 0003AG: Bldg 1160-Dining Hall (Quarterly) 0003AH: Bldg 1350-Main Child Development Center (CDC) (as needed) 0003AJ: Bldg 1525-Child Development Center Annex (monthly cleaning) 0004: Unscheduled/Emergency (On-Call) Services within a 12-month period. Estimated 5-Unscheduled/Emergency on-call services for the Grease Traps: The solicitation document and incorporated provisions and clauses are those in effect through the most current Federal Acquisition Circular (FAC) 2005 51 and are found either in the Federal Acquisition Regulation (FAR); Defense Federal Acquisition Regulation Supplement (DFARS); Air Force Federal Acquisition Regulation Supplement (AFFARS) and/or other Mandatory Procedures/Supplements or Regulations. The following provisions and clauses apply to this acquisition and in order to be considered for this award: 1. Provision at FAR 52.212-1, Instructions to Offerors-Commercial Items (Jun 2008) has been tailored: At the time of quote submittal, All interested parties must be registered and active in the Central Contractor Registration (CCR) Database (www.bpn.gov/ccr). Offers shall include acknowledgment of all amendments as applicable, issued for this solicitation and the following addendums apply to the cited paragraphs in the provision FAR 52.212-1: (a) North American Industry Classification System (NAICS) codes: 562212 and/or 562219 as mentioned above and small business size standard of $12.5 million. (b) Submission of offers: Completed Price Schedule and be compliant with the combined synopsis/solicitation requirements. (c) Period for acceptance of offers of 30 calendar days from the date specified for receipt offers. (e) Multiple offers: are not allowed (h) Multiple awards: will not be made (i) Availability of requirements documents cited in the solicitation: The solicitation and requirements documents are only available from the FEDBIZOPS website. 2. Provision at FAR 52.212-2(a) Evaluation - Commercial Items (Jan 1999): The intent of the solicitation is to achieve the best value for the Government, based on the lowest priced technically acceptable offer IAW the Performance Work Statement (PWS). The Government reserves the right to award without discussions, except for clarifications or resolve minor or clerical errors; and it is in your best interest to include your most favorable terms in your initial submission. The Government does reserves the right to reject any or all offers if such action is in the public interest. The following evaluation factors will be considered-technical characteristics; price; past performance. Failure to submit any of the information requested in this RFQ may lead to the rejection of your quotation without further discussion: (i) Technical Characteristics: The offeror's level of understanding and methodology to accomplish the work in time allotted IAW PWS and provide information that is pertinent to demonstrate the knowledge, competence, capabilities and experience to complete this Solicitation requirement. The offeror's quote should provide sufficient detail to demonstrate a clear understanding of the technical requirements and level of effort needed to accomplish all tasks listed IAW the PWS by providing a current/valid copy of each employee's Commercial Driver's License (CDL) (see PWS, Section 1.6 for special qualification requirement); (ii) Price-Offerors must include prices for each CLIN item (see RFQ Template below). Failure to do so may cause for rejection of the entire offer; (iii) Past Performance, Personnel Qualifications/Experience: The offeror's quote should provide either external reference or contact information for at least one (1) of the most relevant contracts performed for either the Federal agencies or a commercial customer within three years before the date of receipt of proposals. (An offeror that does not have at least one (1) relevant contract shall include a statement to that effect in its proposal). Relevant contracts include those where the offeror performed services of a similar scope, magnitude and complexity as those required under this solicitation. Ensure the following information is included in your reference: Project title and description; contract number, type of contract and amount; Name of the Agency or organization; Name of the Point of contact and a valid telephone number. Therefore, the offeror's initial quote should contain the offeror's best terms; discounts-rebates and price quote including charges or fees and subject to FAR 16.601 (b)(1) and (2) as mentioned above. 3.
