Description
This is not a solicitation announcement and should not be considered as such. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification such as: Women-owned, Hubzone, 8(a), Small Business, Small Disadvantage, Small Veteran Owned Business or Service Disabled Veteran Owned Small Business) relative to NAICS 339113 (Size Standard of 500 employees). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published at www.fbo.gov website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The Veterans Integrated Service Network (VISN) 11 has an ongoing program to furnish artificial limbs and related services to those veterans that require them and are eligible for assistance at the Indianapolis, Illiana HCS, NIHCS, Ann Arbor, Detroit, Battle Creek, Saginaw VA Medical Centers. To create a professional environment in which the Veterans will receive quality products and services. The Contractor shall fabricate all artificial limbs in strict conformance to the prosthetic prescription that has been provided by the VA Amputee Clinic Team. Contractors shall not add or substitute components or alter the beneficiary's limb prescription in any way without prior approval and written authority from the VA Amputee Clinic Team Prosthetic Representative or designee. Appropriate state licensure/certifications shall be required if deemed necessary by the state. Contractor shall provide appropriate professional business license within the states required per each facility serviced. Contractor shall also comply with any other federal, state, and local law applicable to the industry. This documentation shall be provided annually during the life of the contract. Potential sources must demonstrate experience and knowledge of required services and capability to provide such services. The purpose of this notice is to gain knowledge of potential qualified sources and their capabilities. Potential sources should identify business name, point of contact, business size, and any applicable business socio-economic category, i.e. Service Disabled Veteran Owned (SDVO), HUBZone, 8(a), Small Disadvantaged Business (SDB), Woman Owned (WO), and Veteran Owned (VO), service capability statement, provide information for recent and relevant projects similar in nature, DUNS number, address and any additional information felt to be pertinent. The NAICS code for this procurement is 339113 and the small business size standard of 500 employees. Responses to this notice shall be e-mailed, faxed or mailed to attention of Steven D. Rowland, Richard L. Roudebush VA Medical Center, 1481 W. 10th Street (90CSC), Indianapolis, IN 46202. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. THE VA IS NOT SEEKING PRICING OR OFFERS OF ANY KIND. There is no guarantee, expressed or implicit, that the market research for this acquisition will result in a particular set-aside or sole source award, or any other guarantee of award strategy. All information is to be provided on a voluntary basis at no charge to the Government. Any further information will be posted on www.fbo.gov.
