Description

Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. It is the intent of the Syracuse VA Medical Center located in Syracuse, NY, to negotiate a sole source procurement with Northeast Medical Consulting Inc, 57 West Liberty Street, Hubbard, OH 44425 to provide a Powerex, Triplex Oil-Less Scroll Medical Air System (description and capability requirements listed below). 1 Powerex, Triplex Oil-Less Scroll Medical Air System, 5 Hp (ea) motor, 208V, 3 PH, 60 HZ, 34.4 SCFM @ 100 PSIG, 80 Gal Receiver, Desiccant Dryers, CO & Dew Point Monitors, Filtration Package, NFPA 99 Compliant 1 eachTotal Cost ____________ 2 NFPA 99 Certification - not to exceed 8 hours - normal business 1 eachTotal Cost ____________ Description and capability requirements: Provide and certify a Powerex, Triplex Oil-Less Scroll Medical Air System within 5 working days once Purchase Order is issued. No other source can provide required delivery of this urgent & compelling need for the Powerex, Triplex Oil-Less Scroll Medical Air System. The Contracting Activity will consider offerors' quotations submitted by May 4, 2011. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification - Commercial Items, applies to this solicitation. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The offeror shall also provide its Commercial and Government entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of the information added to CCR. To register, go to http://www.ccr.gov/. Offers are due by May 4, 2001 at 1:00 pm EST, received by Steven Tompkins, Contracting Officer, VA Medical Center, Contracting Office, 800 Irving Ave. Syracuse, NY 13205. ELECTRONIC OR FAXED OFFERS WILL BE ACCEPTED with confirmed verbal acknowledgement of receipt by the Contracting Officer.
