Spinal Disc Stimulation Implant Eon Mini IPG Part # 3788 Qty. 1 Octrode Perm Lead 60cm Part # 3186 Qty. 2 Eon Mini Charger Part # 3721 Qty. 1 Programmer Part # 3851 Qty. 1 Octrode Trial Lead 60cm Part # 3086 Qty. 4 Octrode Perm Lead 90cm Part # 3189 Qty. 2 Notice to Contractor: Solicitation number N00259-11-T-0286 is issued as a notice to procure from only one source IAW FAR 13.106-1(b). This proposed contract action is used for Anesthesiology Department/Pain Management Clinic for Neurostimulation Implant, consumables are compatible and specific to the patient of Naval Medical Center San Diego that is schedule for implant on June 3rd 2011. The Naval Medical Center San Diego intends to solicit with only one source under the authority of FAR 13.106-1(b). The source is St Jude Medical, Inc., CAGE Code: 76905, NAICS: 334510. Justification: As required by FAR 13.106-1(b), the Contracting Officer has determined that St Jude Medical devices is the only company that manufactures and distributes the supplies that is compatible with equipment currently used by NMCSD. IAW FAR 5.207(c)(15)(ii), all responsible sources may submit a quotation, which shall be considered by the agency. The Government will only consider firm, fixed price quotations. The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-51. It is the contractor ™s responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov and/ or www.acq.osd.mil.
