- Purpose of this modification is to add the beginning of the notice, before the sole source determination. To extend the closing date to Friday May27, 2011 @7:30 am eastern time. THIS IS A NOTICE OF INTENT, NOT A REQUEST FOR A PROPOSAL. A SOLICITATION DOCUMENT WILL NOT BE ISSUED AND PROPOSALS WILL NOT BE REQUESTED. The COAC Services Branch, Office of Acquisitions, DERA, National Heart, Lung, and Blood Institute (NHLBI), NIH, for the Cardiovascular Pulmonary Branch (CPB), intends to negotiate and award a purchase order on a noncompetitive sole source basis to Dr. Oswaldo Castro, 9128 Orchard Brook Dr., Potomac, Maryland 20854, to conduct research and training in the diagnosis and treatment of vascular diseases, such as pulmonary hypertension in patients with sickle cell disease. The reference number: NHLBI-CSB-(HL)-2011-086-DDC. Background description: Sickle cell is associated with a host of complications, among them pulmonary hypertension. A clinic has been set up at the NIH Clinical Center by the NHLBI Cardiovascular and Pulmonary Branch that supports a large number of sickle cell clinical research protocols in the NHLBI, particularly protocol numbers: 04-H-0161 and 01-H-0088. There is a need for on site and intermittent, on-call phone consultation for clinical patient care advice regarding complicated sickle cell patients with complications related to sickle cell disease and pulmonary hypertension. The questions that arise are highly technical, requiring a comprehensive knowledge of published medical literature in this area, and extensive personal clinical experience in advanced clinical management of complications of pulmonary hypertension in sickle cell adults. Purpose: To conduct research and training in the diagnosis and treatment of vascular diseases, such as pulmonary hypertension in patients with sickle cell disease. Objective: To provide expert consultative advice to medical staff and trainees of Cardiovascular and Pulmonary Branch on the expert medical care of patients with sickle cell disease enrolled on protocols at the Clinical Center, NIH and provide education and training to the staff and trainees of the Cardiovascular and Pulmonary Branch. Contractor Requirements: The contractor shall serve as an expert consultant to the Cardiovascular and Pulmonary Branch/NHLBI. The contractor shall be a resource to credentialed providers seeing patients in the NLHBI/CPB Sickle Cell Clinic and will provide expert and experienced information on the standard of care management of sickle cell disease. The contractor is required to be on-call by telephone for expert advice and consultation 24 hours a day, Tuesday through Sunday of every week. In addition to supporting patient care services, the contractor shall provide education and training to the Cardiovascular and Pulmonary Branch staff and trainees on the subject of pulmonary vascular complications in adult patients with sickle cell disease. The contractor is required to be a licensed physician with at least 100 peer-reviewed publications recorded in Pub Med, documenting international level recognition of clinical and research expertise in pulmonary vascular complications of sickle cell disease. At least 20 years experience in the care of sickle cell patients is required as well as physician board certification in hematology and internal medicine. The contractor shall be available to personally attend weekly NHLBI/CPB Sickle Cell Clinic and to provide personal instruction to staff and trainees weekly, except for planned vacations. The physician must be available when asked to review manuscript drafts and provide constructive input and editing. Delivery of Services: The contractor shall provide patient care support and training services on every Monday. The physician will also be on-call for expert advice 24 hours a day, Tuesday through Sunday of every week. The NHLBI/CPB principal investigators directing branch sickle cell research will be responsible for periodically providing verbal review of the acceptability of support services rendered by the physician. Period of Performance: The period of performance shall be for twelve months after receipt of purchase order. The estimated level effort for this requirement is not expected to exceed 264 hours for twelve months.
