- Contractor's Request for Clarification form Contractor’s Responsibility Questionnaire General Wage Decision CA20100029 03/18/2011 Statement of Work Solicitation Section B-M SF-1442 Request for Quote REQUEST FOR QUOTATION SOLICITATION NOTICE # HSCG31-11-Q-PAA514 1. The U.S. Coast Guard has a requirement for Bldg 50 - Refinish exterior walkways and stairs at Coast Guard Island, Alameda, CA. 2. If you are interested in submitting a quote, please review the enclosed documents. All submittals must be completed and forwarded to this office for consideration of your offer. 3. This solicitation is considered Construction and is conducted as a 100% Small Business Set-a-Side. Contractor shall provide all labor, materials, equipment, transportation, supervision, and all other necessary items for a complete and useable product in full compliance with all plans, drawings, and specifications as defined in the solicitation documents. Estimated Range: $25,000 - $100,000; Davis Bacon Wage Rates apply (copy attached) NAICS: 238190 Small Business Size Standard: $14.0M 4. You must return one complete copy of the entire solicitation package to include (1) The SF-1442, (2) Section B1 - Base Bid, (3) A proposal that is technically acceptable addressing each line item and sub line item in the Scope of Work with a list of proposed materials, construction process, a project timeline, and lead time requirements for product acquisition. (4) A completed Contractor Responsibility Questionnaire, attached, is requested to assist the Contracting Officer in the determination of the contractor's responsibility. The contractor is responsible to review the remaining solicitation, Sections C-M, as these clauses will be incorporated into the final contract. a) Please note that the Due Date for receipt of Quote is 22 JUN 2011 by 4:00 PM PST. E-mail and Fax submission will be accepted IAW Section L.9. b) All questions pertaining to this solicitation must be submitted, in writing, to the Contracting Officer at the address cited in Section G.2 of this solicitation.
