Description
SOURCES SOUGHT The Genera l Services Administration (GSA) Region 10, located in Auburn WA intends to establish a firm fixed priced contract for a Fitness Coordinator services at the Federal Center South government facility located at 4735 East Marginal Way South, Seattle, W A. This will be a performance based contract of one (1) base year and four (4) potential one (1) year option years. Anticipated start date is April 1, 2012. The Fitness Coordinator will be expected to provide a complete, effective, well balanced exercise, wellness, and lifestyle program for approximately 30 -40 hours a week. The Fitness Center located within the Federal Center South facility is approximately 5,500 square feet and has a weight room with stationary machines, cardiovascular machines, multi-station machines, racquetball court, a multi-purpose room (basketball, volleyball courts), and separate men's and women's restrooms/locker rooms. Tasks will include but are not for the purpose of this Sources Sought limited to: •1. Management of the facility to include membership activities, enforcement of the rules, monitoring and maintaining the equipment, surveying the members and marketing the facility and programs with the tenants. •2. Design, coordinate, and administer a complete and comprehensive wellness and fitness program that will include, but not limited to, exercise classes, weight training, nutrition, wellness education, and recreational activities consistent with the membership needs assessment and clients interest. •3. Coordinate all activities with the Property Manager including the submission of the required reports. The fitness coordinator will be required to have in place the necessary education, experience, and certifications requirements as will be specified in the solicitation. Submission Requirements: •1. A positive statement that your firm intends to propose on this acquisition. •2. Previous experience to substantiate capability to perform this type of work. •3. Current HUB Zone certification •4. This is not open to vendors that have one or more negative terminations for cause within the last 7 years performing within the same or similar scope from state or federal agencies. Past Performance Information Retrieval System and the Federal Debarred Vendor list will be verified. State agencies will be contacted individually. Based on the responses received, the Contracting Officer will make a determination whether or not to utilize a HUB Zone set aside for this acquisition. This is not a request for proposal. Any company that misrepresents any of the required information at the time of response to this Sources Sought will receive a negative report of misrepresentation from the PBS Region 10 Contracting Office to be submitted to the Small Business Administration. Previous experience: No more than two pages. e. Has not had a Termination for Cause as stated in requirements number 4: Yes Or No f. HUB Zone certification g. Not to exceed four pages in length. 2. Maintain the order as listed above in your response sheets. 3. All responses are required to be an attachment in Microsoft Word. 4. An authorized appointee signature will be required. 5.
