- Site Visit Reservation Form Price Proposal Roll-up Sheet Technical Evaluation Checklist Past Performance Questionnaire DD 254 Security Classification Collective Bargaining Agreement Wage Determination Performance Work Statement Solicitation The AAC Operational Contracting Division, Eglin AFB., Florida intends to solicit and award a firm-fixed price contract for Transient Alert Services. This is a solicitation for commercial items prepared in accordance with the Federal Acquisition Regulations (FAR) Part 12, Part 13 and Part 15, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. The announcement number is FA2823-11-R-0012. The North American Industry Classification System (NAICS) code for this acquisition is 488190 with a size standard of $7Million. This requirement is a competitive 100% Small Business Set-Aside. Please identify your business size in your response to this standard. The basic performance period is 1 October 2011 through 30 September 2012, plus four one-year option periods. The requirement is for a service provider to supply personnel, equipment, tools, materials, vehicles, supervision, logistics, training, technical support, and other items and services (except those items and services specifically identified as government furnished) necessary to manage and perform Transient Alert support tasks and functions specified within the Performance Work Statement dated March 2011. The service provider shall ensure work, services, and operations are in compliance with local, state and federal laws and regulations, publications and applicable technical orders. The service provider shall comply with safety, security, and environmental requirements. NOTE: A site visit will be held on Wednesday, June 15th at 1:00 p.m. Central Time. Eglin Air Force Base is located directly off Highway 85 in Valparaiso, FL. The purpose of the site visit will be to visit the Flight Line at Eglin AFB and SFG (Special Forces Group) Flight Line at Duke Field, FL. The site visit is expected to last approximately two (2) hours total for both areas, including an approximate driving time of 30 minutes to Duke Field. You are requested to submit your request for attendance using the Site Visit Reservation Form to Ms Donna McDonald by fax (850-882-1680) or email notification (donna.mcdonald@eglin.af.mil) NO LATER THAN 8 June 2011, 2:00 p.m. Central Time, so we can make arrangements for access to the installation. Please limit your number of attendees to two (2). You will be responsible for submitting the Eglin CAB (Contractor Access Badge) Affidavit. Submit NO LATER THAN 8 June 2011, 2:00 p.m. Central Time. This document must be hand-delivered to the Eglin East Gate, or Eglin West Gate Pass and ID Office. You may also mail or FEDEX the documents to: 96Security Forces Squadron, Bldg 272 302 West D Avenue Eglin Air Force Base, Florida 32542 Attn: Pass and Registration We will meet the interested parties for the site visit at the Eglin East Gate Pass and I.D. Office. Questions from the site visit must be submitted in writing within 2 working (business) days after the site visit (17 June 2011, 4:00 p.m. Central Time). Each question shall reference the specific section or attachment of the solicitation (as applicable) and the corresponding paragraph number. Remarks and explanations at the site visit shall not alter the terms and conditions of the solicitation unless the solicitation is formally amended. Any oral statements made by Government representatives shall not be binding upon the Government; all necessary changes will be made in writing by the Contracting Officer. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-50, Effective 16 May 2011. Vendors must be registered in Central Contractor Registration (www.ccr.gov) before award can be made. All questions must be submitted in writing no later than 2:00 p.m. Central Time, 20 June 2011. No questions will be accepted after the closing date. Each question shall reference the specific section or attachment of the solicitation (as applicable) and the corresponding paragraph number.
