Description

AAC/PKOB, Eglin AFB, Florida intends to solicit and award a firm-fixed price contract for non-personal Veterinarian Services. This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) Subpart 12.6 and Subpart 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. This solicitation is issued as a Request for Quotation (RFQ), the announcement number is F1T3CL1097A001. The North American Industry Classification System (NAICS) code for this acquisition is 541940 with a business size standard of $7,000,000. Please identify your business size in your response based upon this standard. This acquisition is 100% set aside for Small Business and will be evaluated based the lowest price technically acceptable (LPTA). Interested parties may be required to meet with Government representatives to discuss the solicitation. The requirement consists of the following: CLIN 0001 Non-personal service contract for one veterinarian at the Eglin AFB Veterinary Treatment Facility. Estimated period of performance is 1 July 2011 to 30 June 2012. Basic Contract plus 4 Option Years Estimated Dates dependent upon contract award: Basic 1 Jul 2011 - 30 Jun 2012 Option 1 1 Jul 2012 - 30 Jun 2013 Option 2 1 Jul 2013 - 30 Jun 2014 Option 3 1 Jul 2014 - 30 Jun 2015 Option 4 1 Jul 2015 - 30 Jun 2016 Veterinarian Duties: Examines, vaccinates, treats and performs authorized procedures on privately owned animals as well as authorized procedures on government owned animals as needed. Interprets laboratory and diagnostic procedures such as routine hematological tests, urinalysis, fecal and heartworm tests. Explains medical findings to animal owners and prescribes treatment. Informs and advises the Gulf Coast Veterinary Commander upon the diagnosis of zoonotic diseases. Maintains proper SOAP format and legible animal health record, controlled drug reports, and prepares other required reports as indicated. Conducts physical examinations prior to issuance of health certificates and release of rabies quarantine suspects. Makes appropriate and proper civilian referrals when indicated. When military veterinarians are not available, provides emergency medical and surgical treatment for military working dogs as required for stabilization until they can be transferred to a contracted civilian veterinary facility or another military installation. Ensures medical test samples are properly prepared and shipped. Provides veterinary oversight and training to assigned clinic employees. Provides the Government representative with a list of recommended vaccines, medications and supplies needed to maintain clinical operations. Conducts animal disease control and eradication programs. Informs Government representative of equipment defects and facility problems. Qualifications: Must have a Doctorate of Veterinary Medicine (DVM, VMD) or equivalent degree from a school approved or accredited by the American Veterinary Medical Association (AVMA) or certified by the AVMA Education Commission for Foreign Graduate. One year of progressively responsible professional experience that shows a demonstrated ability to perform veterinary medical duties independently or successful completion of a veterinary internship or graduate study, in addition to the DMV/VMD. Must have ability to apply veterinary medical knowledge to accurately diagnose, treat and perform surgery on a variety of animals. Must be able to communicate medical findings and recommend treatments with animal owners. Must be able to lift 50 pounds and stand for prolonged periods of time. Must be licensed to practice veterinary medicine in the state of Florida and complete continuing education requirements required to maintain a license to practice veterinary medicine in Florida. Clinic will not provide funding for continuing education. Must be accredited in the state of Florida in order to perform health certificates. Must have a valid and current DEA license. Must have professional liability insurance. Please provide a copy of the following documents with your quote: 1) Certification of a Doctorate of Veterinary Medicine (DVM, VMD) or equivalent degree from a school approved or accredited by the American Veterinary Medical Association (AVMA) or certified by the AVMA Education Commission for Foreign Graduate. 2) Resume documenting one year of progressively responsible professional experience in performing veterinary medical duties independently or successful completion of a veterinary internship or graduate study, in addition to the DMV/VMD. 3) Current license to practice veterinary medicine in the state of Florida. 4) Current DEA license. 5) Valid and current professional liability insurance certificate. Failure to include these items will render your quote as technically unacceptable. Performance Standards: Diagnoses animals accurately and recommends proper treatment. Must be able to provide service for at least four clients per hour. Must maintain professional working relations with the clinic staff and the public. Dispenses medications accurately. Accurately accounts for controlled drugs and medications. Ensures area of responsibility is maintained in a clean, safe condition. Keeps Government representative informed of all problems or potential problems. Notifies the clinic inventory clerk promptly of medical supply shortages and requirements. Terms of Service: Total hours of work per week will range from 24 to 40 hours as needed by clinic requirements; a minimum 24 hours per week. Contract will be for a term of one year. Normal appointment hours will be from 0800 to 1530; hours may change based on clinic needs. Scheduling will be performed by the government a minimum of one week out, some exceptions may occur due to unseen events. Doctor appointments are scheduled every twenty minutes and are subject to change. Veterinarian may also be required to perform examinations on patients being seen by veterinary technicians. On-duty emergencies, non-scheduled patients and "drop-offs" may occur during scheduled duty hours and should be considered as possibilities that occur with any veterinary practice. Veterinarian may be asked to consider working an evening clinic not more than twice a quarter. If unable to work a scheduled shift, the veterinarian must make all attempts to contact the Government representative as soon as possible. Maximum Per Hour Pay will be $46.00. Contract will be awarded to the highest per hour proposal, pending if all the requirements are met.
