- The Health Care for Homeless Veterans (HCHV) Program, located at the North Texas Veterans Health Care System, 4500 S. Lancaster Road, Dallas, Texas, has a requirement to assist with the provision of residential placement and supportive services within the rural areas of Tyler, Texas. At the Contractor's facility, the Contractor shall serve homeless Veterans in need of immediate placement in a safe, transitional housing environment. Services shall be in accordance with the Statement of Work of the contract. With contractual opportunities for an estimate of up to 20 Veterans, a Contractor will be required to: 1.Submit business classification/socio-economic indicator/status, i.e., Small Business, Large Business, Women Owned Business, etc. 2.Provide community-based transitional housing services. 3.Provide clinical case management with supportive services, life skills training, sobriety maintenance and recovery education. 4.Provide counseling services while in a transitional housing setting to aid the Veterans in reaching goals of attaining permanent housing successfully while maintaining recovery and stability. The Contractor would be responsible for providing sufficient residential capacity to place up to 20 Veterans in safe community-based residences. Suitable residential placement generally requires: 1.Sufficient personnel to assure security 2.Staffing available on-site 24 hours a day, 7 days a week 3.Three (3) nutritionally adequate meals a day, 7 days a week 4.Laundry facilities and detergent available for the Veterans to do their own laundry 5.The ability to place a Veteran on the same day of a referral from HCHV staff or Contracting Officer Technical Representative (COTR) 6.Hygiene products to include wash cloths, soap, feminine products, deodorant, toothpaste, and tooth brush shall be made accessible to Veterans 7.Transportation to VA Medical appointments or vouchers for use of public transit 8.Suitable bedroom space and furnishings allowing for adequate storage and security of belongings, with no more than 2 Veterans per room less than 140 square feet The anticipated Period of Performance for these services is August 1, 2011 through June 30, 2012 with four (4) one-year option renewals. The Request for Proposal (RFP) number is VA-257-11-RP-0232. The applicable NAICS code is 624229 and the small business size standard is $7.0 million. The estimated issue date is June 27, 2011, and the anticipated closing date is July 15, 2011 at 3:00:00 PM Central Standard Time.
