Description

DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1.Contracting Activity: Department of Veterans Affairs, VISN 9, 1639 Medical Center Parkway, Murfreesboro, TN, 37129. Regarding transaction 614-11-3-063-0223. This is an emergency order. The period covered is from 13 June 2011 through 13 August 2011 (60 day service). Nature and/or Description of the Action Being Processed: This is a request for other than full and open competition for the procurement of fabrication of removable dental prostheses.This will include full and complete partial dentures or other similar prostheses as needed. These prostheses may be implant-supported. Materials used will comply with the Buy American Act and HIPPA regulations. Each patient case will include a prescription on a form provided by the contractor. The prescription will include enough information for the lab to fabricate the indicated prosthesis. Each prescription with needed materials will be boxed in boxes provided by the lab. The boxes ready for pickup will be in the VA Memphis dental clinic lab and clearly labeled. Pick-up and delivery must occur daily Monday-Friday as needed and will not involve postage or shipping. The working time needed per prescription will not exceed 5 business days unless prior arrangements have been made. Quality assurance will be monitored at both the contracting lab and at VA Memphis. On delivery, cases will be evaluated by the prescribing dentist to ensure that the prescription has been followed. As stated, materials used will comply with the Buy American Act. When possible, materials such as denture teeth and implant components will be provided by the VA dental clinic. The prescribing dentist will include these materials as needed. The contracting lab will also provide any needed materials as prescribed.The point of contact for the contractor with regards to services will be the VA dental lab supervisor. If questions arise in regards to a fabrication process, the technician will contact to prescribing dentist at VA Memphis. Oral Arts Lab does not have a FSS schedule and is an open market procurement. There will be a future RFQ that will be posted to FBO, for a base plus four years for this service, as an SDVOSB set-aside on or about July 5, 2011. Dental has an immediate need to receive services now in order to alleviate backlog of removable prosthodontics. Dental is requesting an immediate purchase order for the immediate needs of the VA patients while the current RFQ is currently being worked on by the CO and the Dental Service.
