- Sole Source Letter This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442711171014; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-42 effective 18 May 2011, Defense DPN 20110518 effective 18 May 2011, and AFAC 2011-0421 effective 14 June 2011. This acquisition is unrestricted; the North American Industry Classification System (NAICS) code is 339113. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 3841. Travis AFB intends to award a purchase order for a Sole Source Baha Cochlear Surgery set. Please see attached sole source letter for futher information. IAW FAR 5.207(c)(15)(ii)- "All responsible sources may submit a capability statement, proposal or quotation which shall be considered by the agency". CLIN0001- 1 BP100 Sound Processor Soft Black (91301), 2 BP100 Sound Processors Chestnut Brown (91303), 3 Baha three system one stage procedure orders. 1 Set Award shall be made in the aggregate, all or none.
