- Description
- The Centers for Disease Control and Prevention (CDC) intends to execute a contract modification to contract 200-2004-07072 National Health and Nutritional Examination Survey (NHANES) with Westat, Inc. to exercise the Community Health and Nutrition Examination Survey (CHANES) option included in the original contract award as follows: I. Background and Need: The NCHS has received funds from the Department of Health and Human Services for a study to collect nationally representative data on physical activity and fitness in children and adolescents. Secretary Sebelius has dedicated funds to Obesity Prevention and Fitness, one of four critical priorities for the Department, in line with the 2010 Affordable Care Act (ACA): Laying the Foundation for Prevention. The Obesity Prevention and Fitness priority advances activities to improve nutrition and increase physical activity to promote healthy lifestyles and reduce obesity related conditions and costs. One out of every three children in the U.S. is now overweight or obese, a condition that places them at greater risk of developing diabetes, heart disease, and cancer over the course of their lives. Regular physical activity in children and adolescents promotes health and fitness and may help to prevent obesity. In October 2008, the federal government issued its first-ever Physical Activity Guidelines for Americans to provide science-based guidance on the types and amounts of physical activity that provide substantial health benefits for Americans. Guidelines for Children and Adolescents ask for 60 minutes or more of aerobic, muscle-strengthening, or physical activity daily. While the number of children in the U.S. who meet the Physical Activity Guidelines is unknown, the percentage of children who are physically active in the U.S. is declining. In the National Health and Nutrition Examination Survey (NHANES) 2003-2006, a physical activity monitor was fielded for ages 6 years and older; in NHANES 1999-2004 cardiovascular fitness was reported as estimated maximal oxygen uptake (VO2max) from a treadmill test for ages 12-49 years; and in 2011, NHANES implemented an upper body muscle strength component for ages 6 years and older. The collection of data on physical activity and fitness in National Youth Fitness Survey (NYFS) for children as young as 3 years of age would provide additional information with which to evaluate changes in physical activity and fitness and weight of children and adolescents over time. II. Purpose and Objectives: The requirement is for a National Youth Fitness Survey (NYFS) dress rehearsal in 2011 to collect and deliver physical activity and fitness data on about 200 children and main NYFS data collection in NYFS 2012 to collect and deliver physical activity and fitness data from about 1,500 children. III. Scope of Work: The Community Health and Nutrition Examination Survey (CHANES) option of the NHANES contract was included to allow the opportunity to extend NHANES examinations to other populations not currently included in the survey. The funds received from the ACA allow the exercise of this option to study aspects of physical activity and fitness in 1500 additional children ages 3-15 via the National Youth Fitness Study (NYFS). The study will include tests of physical activity and fitness selected by National Center for Health Statistics (NCHS) staff with the input of experts in the fields of physical activity and kinesiology. A general description of the tasks and deliverables to be accomplished during the project period is outlined below: Task 1: Conduct a NYFS dress rehearsal in 2011 at two NHANES sites using refurbished trailers. The contractor will furnish necessary labor, materials, supplies, equipment, and services to perform the work set forth below. The contractor will hire, train and monitor the interviewers and health technicians for the dress rehearsal and the NYFS. Work under this contract will be monitored by the Contracting Officer's Technical Representative (COTR) for the CHANES NYFS (Lori Borrud) and COTR for the NHANES (George Zipf). The dress rehearsal will be conducted concurrent with the regular NHANES mobile examination center at the sites, but will not be part of the NHANES. Children will be screened for participation in the dress rehearsal separately from the regular NHANES. The dress rehearsal will include approximately 200 children. The examinations to be conducted will include the following: • Anthropometry (height, weight, and BMI), ages 3-15 • Physical activity monitor (PAM), ages 3-15 • Upper body muscle strength (hand grip), ages 6-15 • Dietary interview, ages 3-15 • Treadmill, ages 6-15 • Gross motor skills, ages 3-5 Other tests may be added based on the results of a feasibility study to be conducted this June and July. A physician, nurse practitioner, or physician's assistant will be present during all examinations in the refurbished trailer. $20 remuneration will be provided to the participants and a $30 incentive to parents, as well as coverage of transportation costs. When the PAM is returned, a remuneration of $40 will be sent to the parent. In addition to the physical examinations, the dress rehearsal will include a household interview to collect socio-demographic information and data on usual physical activity and sedentary behavior. Reports of Findings (ROF) will be provided to parents at the end of the examinations. This will include height, weight, body mass index at a minimum and potentially a measure of aerobic capacity from the treadmill test. The dress rehearsal will utilize NHANES survey methodology and data collection protocols. Task 2: Conduct the NYFS using refurbished trailers at 15 NHANES sites in 2012. The NYFS will be conducted concurrent with the regular NHANES mobile examination center at 15 NHANES sites in 2012, but will not be part of the NHANES. As in the dress rehearsal, participants will be selected in a screening process apart from the regular NHANES. The same interview and examination data collected during the dress rehearsal will be collected on approximately 1,500 children. A physician, nurse practitioner, or physician's assistant will be present during all examinations in the refurbished trailers. The same remuneration, incentive, and transportation costs will be provided. Reports of Findings will be provided to parents at the end of the examinations. This will include height, weight, body mass index at a minimum and potentially a measure of aerobic capacity from the treadmill test. The NYFS will utilize NHANES survey methodology and data collection protocols. Deliverables The contractor will deliver the dress rehearsal and main study data to NCHS on a continuous basis using the current NHANES technology platform for file structure, programs, and routines. Option 1: Conduct the NYFS at 15 NHANES sites in 2013. The NYFS will be conducted concurrent with the regular NHANES mobile examination center at 15 NHANES sites in 2013, but will not be part of the NHANES. Participants will be selected in a screening process apart from the regular NHANES. Interview and examination data collected during the dress rehearsal will be collected on approximately 1,500 children. A physician, nurse practitioner, or physician's assistant will be present during all examinations in trailers. The examinations to be conducted will include the following: • Anthropometry (height, weight, and BMI), ages 3-15 • Physical activity monitor (PAM), ages 3-15 • Upper body muscle strength (hand grip), ages 6-15 • Dietary interview, ages 3-15 • Treadmill, ages 6-15 • Gross motor skills, ages 3-5 Other tests may be added.
