Attachment 11-Sample Design Request for Proposal-Revised Attachment 5-Statement of Work-Revised Section E-Revised Section C-Revised HSCETC-11-R-00001 Amendment 5 SF30 HSCETC-11-R-00001 Contracting Questions and Answers FINAL Attachment 8-IDIQ Pricing Revised The purpose of Amendment 00005 to HSCETC-11-R-00001 is to: 1. Change the due date for proposals to 6 July 2011 at 3 pm Eastern Daylight Time (EDT). 2. Provide responses to contracting related questions submitted by potential offerors on 6 June 2011 (See Attachment titled 'HSCETC-11-R-00001 Contracting Questions and Responses FINAL'. 3. Inform potential offerors that as a result of the technical and contracting questions/responses, RFP no. HSCETC-11-R-00001 (including Section C, Section D, Section E, Attachment 5-Statement of Work, Attachment 8-IDIQ Pricing, and Attachment 11-Sample Design Scenario Request for Proposal) have been revised and are attached to this amendment. 4. Inform potential offerors that corrections to Technical Questions/Responses no. B.9, B.10, and B.19 that were posted in Amendment 0003 are provided in Attachment titled 'HSCETC-11-R-00001 Contracting Questions and Responses FINAL'. END OF AMENDMENT 00005.
