Description
Description: The 711 HPW’s USAF School of Aerospace Medicine (USAFSAM) requires a high-volume DVD and CD duplicator system to use in reproducing instructional materials for students. This requirement is in conjunction with USAFSAM’s BRAC-related relocation to WPAFB. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The Request for Quote (RFQ) number is FA8604-11-Q-7456. This solicitation and incorporated clauses/provisions are those in effect through Federal Acquisition Circular 2005-52 Effective 31 May 2011 and Class Deviation 05o0001. Defense Federal Acquisition Regulation Supplement DCN 20100927. This is a brand name or equivalent for Rimage Disc Copy Station and Everest Printer. The Offeror shall only provide a quote for items listed in attached requirments. For the ease of reviewing all quotes, please keep all part numbers in the same order as listed. Offeror must be an authorized brand name or equal reseller and certified service provider of items listed. Offeror must quote on all items or none. THE AIR FORCE REQUIRES THAT RESPONDENTS PROPOSE OPEN MARKET PRICING ONLY; SMALL BUSINESS ONLY; BRAND NAME OR EQUAL. This Solicitation incorporates by reference the following FAR and DFAR clauses: 52.204-7, 52.212-1, 52.212-3, 52.212-4, 52.212-5, 252.204-7004- Alternate A, 252.211-7003, 252-212-7001, 252.227-7013, 252.227-7014 and 252.212-7000. The below clause is included in full text: AF 5352.201-9101 OMBUDSMAN (APR 2010) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (c) If resolution cannot be made by the contracting officer, concerned parties may contact the Center/MAJCOM ombudsmen, Howard E. (d) The ombudsman has no authority to render a decision that binds the agency. (e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the Contracting Officer. Please note if manufacturer requires a license agreement to be signed by government, the terms and conditions of that agreement are required to be in compliance with all Federal Laws, Statutes, and the Federal Acquisition Regulation. One year warranty that includes next business day delivery of replacement unit or parts.
