Description
1.0 ABSTRACT This item describes the work to be accomplished by a FASSMER Technical Representative to conduct an annual inspection of the port ladder and install and test the new starboard accommodation ladder manufactured by FASSMER. 2.0 REFERENCES: Work item for Shipyard installation of new ladder. 3.0 ITEM LOCATION AND DESCRIPTION 3.1 Location/Quantity: USNS Matthew Perry (T-AKE 9). VR Location: Guangyang, Republic of Korea. 4.0 GOVERNMENT FURNISHED EQUIPMENT/ MATERIALS/ SERVICES: None 5.0 NOTES: 5.1 Access to MSC ships is IAW the MSFSC Contractor Access clause. 5.2 Information required: 1. U. S. Citizen Contractors: Full Name; Full SSN; Ship Attending; Reason For Visit; Dates Requiring Access. 2. Non-U.S. Citizen Contractors: Full Name; Date Of Birth; Nationality; SSN, Passport No. or ARC No.; Ship Attending; Reason For Visit; Dates Requiring Access. 5.3 Period for Planning: 03 August to 23 August 2011 5.4 Fassmer to provide Technical Representatives safety gear and necessary test equipment including weight bags. 6.0 QUALITY ASSURANCE REQUIREMENTS: None Additional 7.0 STATEMENT OF WORK: 7.1 Technical Representative shall provide guidance and direction to shipyard for the installation and testing of the new accommodation ladder. 7.2 Fassmer to provide required water weight test bags. If the water bags are not available locally in the VR Location, see paragraph below. 7.2.1 If water weight test bags are not available locally, they may be shipped to BATS warehouse for forwarding to the VR Location. It is the contractor's responsibility to ensure properly packaged and that the bags are available on time. MSC BATS WAREHOUSE 9284 BALBOA AVENUE SAN DIEGO, CA 92123 POC: LARRY JACKSON, 619-553-6053 7.3 Technical Representative shall conduct an annual inspection of port accommodation ladder. Technical Representative shall conduct weight test of installed and conduct all regulatory requirements for the new starboard accommodation ladder. Labor will be provided by shipyard. Certificates of the successful testing shall be provided to the ship. Fassmer to provide and install new label plates with all applicable information. 7.4 Once all work has been completed FASSMER Technical Representative shall inform the Chief Engineer, Captain and MSCREP on work accomplished provide electronic (PDF) copies of a thorough completion report. 7.5 MSC will return ship the water test bags to BATS warehouse in the original shipping container. 8.0 GENERAL REQUIREMENTS: None
