- The West Palm Beach VA Medical Center (WPB VAMC) located at 7305 North Military Trail West Palm Beach, FL 33410, intends to issue a Full and Open Request for Proposal (RFP) VA-248-11-RP-0061, seeking a single award for a firm fixed price indefinite quantity (IQ) contract for the delivery of Primary Care and Mental Health Services to eligible beneficiaries of VA Medical Center in their local communities in accordance with all terms, conditions, provisions, statement of work, specifications and schedules of the solicitation. Each offer should contain the offeror's best terms from a cost/price and technical standpoint, as contract award may be based on initial offers received without discussions. Communities served under this contract are the Southern Palm Beach County, St. Lucie County, Martin County, Indian River County, and Okeechobee County. The clinics shall be located in Delray Beach, Boca Raton, Fort Pierce, Stuart, Vero Beach and Okeechobee. The period of performance will be for one (1) year base with four (4) One (1) year options to extend, at discretion of the Government. The contract is subject of the availability of funds. The contractor shall provide for the continuous delivery and management of primary care & mental health services for all assigned VA patients. Referrals for specialty care, extensive diagnostic work-ups and non-emergency hospitalizations shall be made to the VAMC. As of 5/30/2011, total enrollment of veteran in each current CBOC is as follows: Okeechobee 1,978 Stuart 5,345 Delray Beach 8,475 Vero Beach 3,278 Fort Pierce 4,202 Boca Raton 3,332 There are an estimated 127,000 veterans residing in the areas covered by this contract. Age demographics are: AgeAgeAge <45 45-6465+ Palm Beach County 65, 20013%34%53% St. Lucie County 23,712 15%36%49% Martin County 18,94211%36%53% Indian River County 15,58311%33%56% Okeechobee County 3,445 20%40%40% Approximately 4% of this population is female. The most common outpatient diagnoses treated include diabetes mellitus, hypertension, atrial fibrillation and atherosclerotic heart disease. The most common DRGs for patients moving to the acute care setting are congestive heart failure, chronic obstructive pulmonary disease, atherosclerosis, angina, and cardiac arrhythmia and conduction disorders. VA estimates that each patient will make three visits per year to the contract primary care facility. This amount is an estimate and is to be used for information purposes only. The contractor shall provide health care providers, medical facilities, medical equipment and supplies, specified pharmaceuticals, and the clinical and administrative functions and support necessary to meet the levels of care and range of services required and/or specified by VA, the VA Medical Center or other governing bodies, agencies and/or directives/mandates. Delivery of care, services and/or goods under this Performance Work Statement shall be in accordance with Joint Commission on Accreditation of Healthcare Organizations (JC) and/or all other regulatory bodies or agencies. All Physicians (including subcontractors), Physicians Assistants and Nurse Practitioners must meet all credentialing and privileging requirements established by the VAMC Medical StaffBylaws prior to commencement of contract work. All staff working under this contract shall be subject of a background investigation and must be initiated prior to commencement of contract work as will be indicated in the solicitation. All work is to be performed in accordance with the guidelines established by Federal, State and local ordinances, and with all terms, conditions, provisions, schedules and specifications provided in the solicitation. The requirement will be follow the implementation of VA for a patient-centered medical home (PCMH) model at all VHA Primary Care sites which is referred to as Patient Aligned Care Teams (PACT). This initiative supports VHA's Universal Health Care Services Plan to redesign VHA healthcare delivery through increasing access, coordination, communication, and continuity of care. PACT provides accessible, coordinated, comprehensive, patient-centered care, in team based environment including the active involvement of other clinical and non-clinical staff. PACT allows patients to have a more active role in their health care and is associated with increased quality improvement, patient satisfaction, and a decrease in hospital costs due to fewer hospital visits and readmissions. Solicitation No VA-248-11-RP-0061 will be electronically issued in accordance with FAR Part 15 on or about 08-01-2011. The NAICS code for this procurement is 621498 and a Size Standard of $10M.
