Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. The Government may or may not issue solicitation documents. The Department of Veterans Affairs, Network Contracting Activity is currently seeking firms, Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, 8(A), other Small or Large Business's with the ability to provide a VISULAS Trion VITE equipment (NAICS Code: 423450) and components listed below for the Northport VA Medical Center located at 79 Middleville Road, Northport, NY 11768-2200. 1.VISULAS Trion VITE with table 2.LIO for Trion and 532s, 5m, with case 3.DIGICAM set 58 without beam splitter - to include DIGICAM Adapter/Illuminator 4.Two copies of instructions covering care maintenance and repair, two sets of spare parts list; electronic versions accepted. 5.TRADE-IN DISCOUNT: LUMENIS COHERENT LASER/MODEL: NOVUS OMNI/SN: Z2036A3 6.TRADE-IN DISCOUNT: ZEISS/MODEL: SL130 SN: 832670 The firm will be responsible for providing all management, supervision, labor, materials, supplies and equipment, and shall plan, schedule, coordinate and assure effective delivery of the equipment. Firms desiring consideration shall email directly to the Contracting Officer referencing the following in the subject field, "RFQ #VA-243-11-RQ-0450 VISULAS Trion VITE", then contain the following information within the body of the e-mail name of firm, Socio-Economic category of firm, complete address, phone & fax number of contact.
