Description
This is a Presolicitation Notice for VA-249-11-RP-0073, ER Physician Services. A Sources Sought notice was previously released under VA-249-10-RI-0093. The JAMES H. QUILLEN VA MEDICAL CENTER, PO Box 4000, Mountain Home, TN has a requirement for approximately 3.86 FTEs Emergency Room Physicians (on-site) and additional approximate 1.50 FTE of Mid-Level Practioners to provide 24 hours/7 days week (excluding two eight hour shifts per week) coverage for the Emergency Room. The ER Physicians must be Board Certified in Emergency Medicine. The services to be performed by the Contractor shall be under the supervision of the VAMC Emergency Department's Medical Director in conjunction with the Office of the Chief of Staff. The proposed contract will be for a base period of on or around April 1, 2012 through March 31, 2013 with four (4) option years exercised at the Government's discretion. A Firm Fixed Price with single award is proposed at this time. The JAMES H. QUILLEN VA MEDICAL CENTER ED is currently a level 3 trauma facility that serves 45000 (2009) eligible veterans in the eastern portion of the VISN 9 area. Patient workload varies significantly throughout the department, depending on the shift and day of the week. Mondays and the days following Federal holidays are historically the heaviest days for ED utilization. Roughly 25000-26000 Veterans triage-in to the Emergency Department annually. The ED consists of seven critical treatment beds (designated "Emergency Room") and six urgent treatment rooms (four designated "Emergency Clinic and three committed to ED Psychiatry service). JAMES H. QUILLEN VA MEDICAL CENTER provides professional coverage by RN and LPN staff. Police/Security is on site twenty-four hours a day, seven days a week (24/7). All patients are triaged using the 5-tier category system, the standardized and nationally mandated Emergency Severity Index (ESI). During normal clinic hours, walk-in patients that have an assigned primary care provider and that do not require "emergent" or "urgent" care are treated in their respective clinics when appointments are available. During non-clinic hours, the ED provides urgent care to eligible veteran patients as their condition requires. The primary goal of the ED is to provide safe optimal care for patients presenting with medical and psychiatric emergencies. This requires careful attention to individual medical problems, efficient management of patient flow, and utilization of facility clinical guidelines where available, e.g. contract and mid-level providers (defined as Physician Assistants and Nurse-Practitioners) are expected to participate in facility-wide collection of performance measure data as appropriate in the ED setting. ED physicians and mid-level providers are expected to communicate effectively with house-staff, Fellows, Attendings, and other providers to facilitate patient dispositions, admissions, and general patient flow. It shall not be necessary for the ED physician to write admitting orders under ordinary operating conditions.
