Attachment 2: APPLICATION OF HULL IDENTIFICATION MARKINGS SOW Attachment #1: USCGC MLB 47280, QA-1: Quality Assurance Inspection Form-Preservation Checklist Scope of Work (SOW) entitled: Underwater Body Preservation Pertaining to USCG Boat MLB 47280, dated July 20, 2011. This Modification/Amendment #2 to Solicitation Number HSCG3716-Q-SP016 is hereby issued to change the following: 1) Modification of Scope of Work (SOW) entitled: Underwater Body Preservation Pertaining to USCG Boat MLB 47280, dated June 06, 2011. Changing the Paint requirements for Line Item #1 Underwater Body Preservation; add the additional scope of work for Line item #2 to repaint the Stripes and Installation of Decals in accordance with the attached SOW for Underwater Body Preservation Pertaining to USCG Boat MLB 47280, dated July 20, 2011. 2) Add the additional documents as attachments to the Statement of Work (SOW) as follows: Attachment #2: APPLICATION OF HULL IDENTIFICATION MARKINGS Attachment #3: 47B-MLB 631-010 Rev. G; 47 FT MOTOR LIFEBOAT HULL VISUAL IDENTIFICATION sheet 1 of 2. Attachment #4: 47B-MLB 631-010 rev. G; 47 FT MOTOR LIFEBOAT HULL VISUAL IDENTIFICATION sheet 2 of 2. 3) Change the Period of Performance from August 18, 2011 - August 25, 2011. To: The new Period of Performance August 18, 2011 - August 31, 2011. 4) A Pre-Quote Conference was conducted on Tuesday, July 12. 2011. However, the USCG shall make accommodations for contractors to see the MLB 47280 and review the requirements by Tuesday, July 26, 2011. Please contact the Contracting Officer to make arrangements. 5) The Quotation Due Date is changed from 5:00PM, July 20, 2011: to 5:00 PM, July 29, 2011. 6) Modified SF-1449-Contractor Quote Sheet reflecting the two line items is attached to this Request for Quotation. 7) Define the requirements for Technical Experience and Technically Acceptable Competence required for consideration for award: It is required that the contractor and also each crew member of the contractor who shall perform these contract requirements have a minimum of three previous successful contract completions for Underwater Body Preservation or similar exterior Hull Painting for U.S. Coast Guard or U.S. Navy vessels. These three contract completions must be recent within the past three years. All contractors submitting a price quotation must also provide documentation demonstrating previous successful contract efforts and Technical Experience performing Underwater Body Preservation or similar scope of work for exterior hull painting of US Naval or Coast Guard Vessels. Contractors are required to identify these previous contracts, the agency issuing the contract, summary of the Scope of Work, Point of Contact and Contact Information (phone and e-mail address) for the Contracting Officer who issued and administered each contract effort. The Quotation Due Date is changed from 5:00PM, July 8, 2011; to 5:00PM July 20, 2011. SF-1449-Contractor Quote Sheet, is hereby attached for contractor quotation submittal. 1) Pre-Quote Conference is scheduled for 11:00AM, Tuesday, July 12, 2011. Within two days of the scheduled Pre-Quote Conference, the prospective contractors planning to attaend, shall provide via e-mail, the names of the attendees to the Contracting Officer. 2) All Quotations must be received by 5:00PM, Wednesday, July 20, 2011. It is requested that all quoptations be submitted on the attached SF-1449-Contractor Quote Sheet. 3) SF-1449-Contractor Quote Sheet is attached at Attachment #4 to this Request for Quotation. Any questions regarding this Modification/Amendment #1 to this combined Synopsis/Solicitaiton or this request for Quotation in general, should submit their questions in writing to the Contracting Officer (Tom Landry) via e-mail (Tom.Landry@uscg.mil). End-Modification/Amendment #1 _________________________________________________________________________ _________________________________________________________________________ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. Solicitation Number HSCG3716-Q-SP016 is issued as a Request for Quotation (RFQ). This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation shall not be issued. Quotations shall be valid for a period of 60days. The period of performance for this contract is August 18,2011 - August 25, 2011. The place of performance is at US Coast Guard Base Terminal Island, Base Support Unit (BSU) San Pedro, 1001 S. Seaside Avenue, San Pedro, CA 90731. This requirement is set aside for small business with a NIACS Code 336611. The objective of this requirement is for Small Boat Underwater Preservation of USCG Boat MLB-47280. In particular: Provide all labor, materials and equipment for High-Ultrahigh-Pressure Water Jetting and necessary surface preparation as reflected in the attached statement of work (SOW) and defined in SSPC-SP-12. Coat the prepared surfaces with the appropriate Sherwin Williams paint in accordance with COMDTINST M10360.3 and as specified in the attached Statement of Work (SOW) entitled Underwater Body Preservation Pertaining to USCG Boat MLB 47280, dated June 06, 2011 and referenced in US Coast Guard Drawing 47B-MLB 801-10, Rev K; Booklet of General Plans. All applicable general requirements and specification documents are attached. The contractor shall perform all work as defined in the statement of work, technical specification, drawings, Society for Protective Coatings Specification and US Coast Guard Commandant Instruction Manual. A Pre-Quote Conference is scheduled for 11:00AM, Tuesday, July12, 2011. Within two days of the scheduled Pre-Quote Conference, the prospective contractors planning to attend, shall provide the names of the contractor personnel intended to attend, to the Contracting Officer (Tom Landry at 310-521-6115) for coordination with the Base Guards. All prospective contractor representatives shall have available and provide to the Gate Guards proper valid identification, vehicle registration and current insurance. Quotes shall be evaluated based on a "Lowest Price Technically Acceptable" basis. The Walsh-Healy Contract Act applies to this solicitation and subsequent resulting contract. Wage Determination, WD 05-2048 (Rev. -13) is attached to this solicitation for your reference. Check the https://dol-esa.gov/SF98 for the current wage determination information from the Department of Labor. The solicitation documents, incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-52.
