Attachment F Attachment E Attachment D Attachment C Attachment B Attachment A Solicitation FBO Modification/Amendment 1 - The purpose of FBO Modification/Amendment 1 is to upload the solicitation and associated documents. The bid due date is also hereby extended to August 26, 2011 per the solicitation. Please note, a pre-bid site visit is scheduled for August 9, 2011 at 9:30 at 1170 4th Avenue South, Park Falls, WI 54552. This concludes additional information added via FBO Modification/Amendment 1. The Chequamegon-Nicolet National Forest announces an opportunity for the Reconstruction of Forest Road (FR) 390. It is anticipated that this acquisition will be advertised as a HubZone set-aside and any resultant firm-fixed price contract award will be issued pursuant to FAR Part 19. In accordance with FAR Part 36.204(c), the estimated magnitude of construction is between $250,000 and $500,000. The estimated contract period of performance is 165 calendar days after the Notice to Proceed. The applicable North American Industry Classification System (NAICS) Code is 237310. The Small Business Administration size standard is $33.5 million. Contractors are required to be actively registered with the Central Contractor Registration (CCR) system and the Online Representations and Certifications Application (ORCA). The successful offeror shall be required to submit Performance and Payment Bonds (100% each) prior to start of work. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS SYNOPSIS. No reimbursement will be made for any costs associated with providing information in response to this synopsis, the pending solicitation or any follow up information requests. The Invitation For Bids (IFB) will be posted to the FedBizOpps website on or about July 7, 2011. No hard copies of the solicitation, supporting documents and applicable amendments, if any will be provided. To receive the entire solicitation package and any applicable amendments, prospective offerors must download them from the FedBizOpps website. The contractor shall provide all management and project administration services, quality control, logistical support, vehicles, labor, equipment and materials to complete the project on time and on schedule. The scope of work is to reconstruct 2550 feet of Forest Road 390 (Quarry Road), replace the existing stream crossing culvert at Trout Brook with a timber bridge structure on concrete abutments, widen existing roadway, blast designated areas of rock, install 10 cross drain culverts, create 6 ditch dams and settling basins, furnish and place aggregate to final road grade; install permanent object marker signs; seed and mulch all disturbed areas; and protect the Trout Brook River and associated wetlands from siltation and other destructive impacts of construction.
