Description
DENTAL SERVICE BID SPECIFICATIONS •A. SERVICES REQUESTED Position Weekly Hours Period of Performance Dentist 16 10/01/2011 - 9/30/2013 The Center Director shall have the authority to request that any contractor-assigned staff member be replaced, with just reason or cause. •B. PERIOD OF PERFORMANCE Offeror shall submit annual price quotes for the base period of 2 years, October 1, 2011 through September 30, 2013, as well as three (3) one-year extensions. Each extension year will be awarded based on an annual performance review, contingent on receiving Department of Labor Approval of funds for payment of services. In accordance with the directions of the National Office of Job Corps, yearly cost increases will be predicated upon Department of Labor Direction. •C. STATEMENT OF WORK The Dental services to be provided will include, but not be limited to those medical services enumerated in the Job Corps Policy Requirements Handbook (PRH) Chapter 6 and appendices. •D. QUALIFICATIONS The dentist serving the Center must be licensed to practice in the State of New Mexico. A copy of their current license(s) will be provided if they occur during the lifetime of the contract. The dentist will have the knowledge and ability to deliver all services normally available in general dental practitioner's office. These include: * dental health education * preventative dental care * diagnostic services, including x-rays * simple surgical procedures * restorative procedures * routine endodontic procedures * prosthetic services * routine periodontic treatment •E. SCOPE OF DENTAL SERVICES • 1. The services of the dentist will be as follows: •a. Oral examinations, emergency treatment and routine dental care, to average a maximum of sixteen (16) hours per week on a schedule to be mutually agreed upon. •b. All routine care will be performed on site. Emergency care may be performed on site or at the dentist's private office. Care will be of the type needed and according to the priority system listed in the Job Corps Regulations ( P.R.H.-6,) available for review at the Center. •c. Provision of a routine dental examination of the American Dental Association Classification, Type II, including bite-wing x-rays, between the 45 th and 75 th days after the student's arrival on Center. •d. Provision of necessary preventive dental health procedures, e.g., instruction for brushing teeth, which will promote the oral hygiene of all students. •e. Provision of an appropriate referral system to ensure the receipt of special care as defined by P.R.H.-6 •f. Provision of coverage by a qualified substitute, approved by the Center, during the absence of the dentist, with any secondary subcontracting in accordance with P.R.H.-6, and approved by DOL. •g. Provision for 24-hour emergency coverage. •h. A diagnosis and treatment plan for each student within two (2) weeks after completion of the routine examination. The plan should include a listing of the teeth involved and specific services to be rendered. 2. The development, implementation and monitoring of the total Center dental health program will include, but not be limited to the following: a. Advise to the Center Director, Center physician and Health Service staff in all dental or dental related matters b. Provision for assistance in developing the coordination and integration of the dental health program with the other health components and other center activities. c. Maintenance and confidentiality of all required dental records. d. Identification and utilization of the most appropriate and least expensive sources for obtaining necessary dental pharmaceutical supplies. e. Assistance in the development and implementation of the dental education component of the health education. f. Issuance and maintenance of signed and dated annual standing orders for dental care in accordance with P.R.H.-6. •E. PAYMENT FOR SERVICES The center will pay the independent contractor upon presentation of a monthly itemized invoice for services rendered. Any third party payments will be credited to the center. The offeror will provide in detail price quotes for the base period of October 1, 2011 through September 30, 2013, as well as yearly extensions. Each extension will be based on an annual review and contingent upon receipt of Department of Labor approval for payment of services. •F. TERMS OF AGREEMENT The independent contractor agrees to adhere to the policies, procedures and guidelines established by Job Corps. It is mutually agreed that the dentist will have primary responsibility to implement the program outlined above. The dentist will be assisted in the development, implementation and operation of the dental program by the Supervisor of Health Services, The Center Director, and the Regional Dental Consultant who will act under guidance and direction from the Medical Director, U.S. Department of Labor, Job Corps. Washington D.C. G. TERMS AND CONDITIONS Under the agreement, the individual or dental subcontractor shall be an independent contractor and not an employee or agent of DEL-JEN, INC. or the Department of Labor, Job Corps. The contractor agrees to indemnify and save harmless the Center and the U.S. Department of Labor, Employment and Training Administration from any and all judgments, orders, awards, cost and expenses, including attorney's fees, and also claims on account of damages to property or bodily injury, including death, which may be sustained by themselves or employees in connection with work performed. The indemnification shall be through provision of a certification of professional liability insured in the minimum of $1,000,000. The agreement may be terminated at any given time by either party upon a thirty (30) day written notice to the other, effective thirty (30) days after acknowledging receipt of the notice. The independent contractor agrees to make no public statements concerning Job Corps students or Job Corps activities without prior written approval of the Center Director or the designated representative. All research and/or publications relating to Job Corps health activities will be cleared through the Center Director, and the Health Office, U.S. Department of Labor, Job Corps, Washington, DC. The agreement will contain the entire agreement between the parties. It may not be modified or added to except by a written instrument signed by both parties. The agreement will, in general, conform to the terms and conditions of this Invitation For Bids (IFB) with such additions and/or modifications as are mutually agreed upon by the two parties. The agreement is subject to approval by the U.S. Department of Labor. H. CONTRACT BIDS Bids must be submitted to the Purchasing Agent or Director of Administrative Services by August 26, 2011.
