Description
RECOVERY - PER FAR 5.7 THIS NOTICE IS PROVIDED FOR INFORMATION PURPOSES ONLY; THEREFORE FAR 5.203 DOES NOT APPLY. THIS OPPORTUNITY WAS AVAILABLE ONLY TO CONTRACTOR(S) UNDER: VA HOSPITAL HVAC RENOVATIONS DT007 AHU-4 New Interferences Removal A. The contractor is to relocate the interferences for the AHU-4 ducts that have been installed as part of the FY09 VA Boiler project. This will include, but not be limited to: 1. Conduit and wire in and out of ATS-2 2. Conduit and wire in and out of Panel EB-E-1 and associated transformers 3. Sump piping and associated vent piping 4. Trap primer piping B. This modification does not add the existing domestic water pipe, steam pipe, condensate pipe, sewer pipe, vent pipe, compressor air supply, vacuum generator exhaust, etc. that was present at the time of the bid. DT044 Existing Louver Openings 1. The Contractor shall demolish the existing louvers listed below. This demolition will include the lime stone sill (if any), louver, duct and all associated hardware. 2. The Contractor shall install a reinforced CMU wall in this opening with a brick exterior that matches the surrounding brick. The contractor shall "tooth-in" the new brick to the exterior brick. A. 2nd Floor Dietetics Kitchen (x2) B. 2nd Floor Dish Room (x2) C. Penthouse B (x1) DT045-2 Dietetics Light Fixtures (A) The purpose of this change is to incorporate Government directed changes into the contract as a 2 part modification with this change being part 2. (B) Fully complete the scope of work in change DT045-1, contract modification P00012: "Remove the requirement to store, clean, replace lamps and ballasts, and re-install existing light fixtures in Dietetics. The existing fixtures shall be turned over to the VAMC. 2'x2' Metallux Parabolic and Metallux Acrylic Lens light fixtures, or equivalent, shall be provided and installed in the locations shown on sketch DT045-SK1. The VAMC will provide 37 lensed light fixtures, Cooper Metalux, QS-2GC8-2U6T8A-UNV-EB81-U. The contractor shall provide 40 prismatic light fixtures and 60 parabolic light fixtures." DT046-2 VA Badge Requirement (A) The purpose of this change is to incorporate Government directed changes into the contract as a 2 part modification with this change being part 2. (B)Fully complete the scope of work in change DT046-1; contract modification P00015. Scope of work from change DT046-1; contract modification P00015: Section 01 00 00 (General Requirements), Paragraph 1.5.C shall be replaced with the following: "C. All employees of the General Contractor and all Sub-Contractors, no matter at what tier, scheduled to be assigned to this project for a total of one hundred eighty (180) days or longer during the total duration of this contract shall be required to be issued a VA photo identification badge to hold in their possession until their assignment to the project is ended, SIGN-IN at the General Contractor's site office daily, and wear a VA Medical Center photo identification badge while on the premises and in the buildings of the Aleda E. Lutz VA Medical Center. The badge will be worn in such a manner as to be visible at all times, and will be located between the waist and the neck on the front of the wearer. Any employees not having a badge shall be ordered to leave the medical center site immediately. i. To receive a VA Photo Identification Badge the contractor must: Step 1: Contractor fills out Form OF 306 (Declaration for Federal Employment) and provides height, weight, race, gender, hair color, eye color, city and State born. This information is sent to Marcie Main [Marcie.Main@va.gov] to enter the contractor in the system. Step 2: Contractor has finger prints and a picture taken by VAMC Human Resources personnel in Building 30 (Beth Keyser). The contractor also fills out Form OF 612 (Application for Federal Employment) or submits a Resume which is given to VAMC Human Resources when finger prints and a picture are taken. Step 3: Contractor fills out Form SF 85. The contractor can either fill out SF85 as an online form or they can fill out the hard copy and submit it to VAMC Human Resources. To fill out the online form, the contractor must provide an e-mail address so the site information can be sent to them."
