- Attachment No. D3 Attachment No. D.2 Attachment No. D.1 RFQ No. 2011-Q-13818 This is a combined synopsis/solicitation for commercial items in accordance with Subpart 13.5-Test Program for Certain Commercial Items, prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 2011-Q-13818 and it is issued as a request for quotation. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-52. This requirement is unrestricted and only qualified offerors may submit responses. The North American Industry Classification System (NAICS) code for this procurement is 334516 and the small business size is 500. The Centers for Disease Control and Prevention (CDC), Center for Global Health (CGH), Division of Parasitic Diseases and Malaria (DPDM), Entomology Branch (EB) has a need to acquire the purchase of one MicrOTOF II Focus Basis System and software or equivalent to. Delivery shall be no later than 45 days from the award date and the FOB Point shall be Destination, Atlanta, GA. The following FAR provisions and clauses apply to this acquisition which may be found in the attached solicitation document: FAR 52.212-1, Instructions to Offerors Commercial Items; FAR 52.212-2, Evaluation Commercial Items; FAR 52.212-4, Contract Terms and Conditions Commercial Items; and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items. By submission of an offer, the offer acknowledges the requirement that prospective awardees shall have posted their FAR 52.212-3 Offerors Representations and Certifications Commercial Items at website http://orca.bpn.gov. Offeror must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov. The award shall be made on a best value basis. The award evaluation will be in accordance with FAR 13.106-2 and will be based on price and delivery schedule. Additional information regarding this solicitation may be obtained from the solicitation document attached to this combined synopsis/solicitation. Offers shall be submitted electronically no later than 2 PM EST, August 9, 2011 to Betty Vannoy, Sr. Contract Specialist, bbv9@cdc.gov, email subject line: RFQ 2011-Q-13818. Questions regarding clarification of solicitation requirements shall be submitted in writing to the Sr. Contract Specialist by email at bbv9@cdc.gov. It is requested that all questions be received by 2 PM EST, August 3, 2011 as time may not permit responses to questions received after that date to be prepared and issued prior to the offer due date/time. The Government will only respond to written questions. All responsible sources may submit an offer that will be considered by this Agency.
