- AWARD NOTICE: The following combined synopsis/solicitation VA-263-11-RQ-0254 has been awarded to American Environmental Pest Control INC, Sioux Falls, SD, in the amount of $5,800.00 for items No. 1 and 2 for base period 8/1/2011 - 7/31/2012. This is a combined synopsis/solicitation for the commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation VA-263-11-RQ-0254 is being issued as a Request for Quotation (RFQ) and incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2005-52. This Procurement shall be solicited under the Simplified Acquisition Procedures (SAP) and the North American Industry Classification System Code (NAICS) code is 561710 with standard size of 7.0 million. These services will be performed under the Service Contract Act Wage Determination 2005-3025, Rev. 13 (6/13/2011). The VA Health Care System, VISN 23, VA Medical Center, Sioux Falls, South Dakota, has the requirement to procure Pest Control Services per the following specifications for Base Year (8/1/2011-7/31/2012) and 4 Option Years (please quote all 5 years): A. STATEMENT OF WORK -PEST CONTROL GENERAL: The contractor shall provide all management, laboratory testing, tools, supplies, equipment, transportation, and labor to perform the tasks associated with the facility ™s Integrated Pest Management (IPM) plan for the interior and exterior of the Sioux Falls Veteran ™s Affairs Health Care System ™s facility (herein referred to as SFVAHCS) in a manner that ensures the health and general well-being of patients, staff, and visitors. All infestations will be addressed until complete elimination of pests. B. LOCATIONS OF SERVICES PROVIDED: All buildings associated with the campus of the SFVAMC. C. WORK HOURS: 1. Normal Work Hours: The service schedule will be developed between the contractor and Contractor ™s Technical Representative (COTR) prior to any service being performed. 2. National Holidays: The ten holidays observed by the Federal Government are: New Years Day, Martin Luther King ™s Birthday, President ™s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran ™s Day, Thanksgiving Day and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. D. SPECIFICATIONS: 1. The contractor shall conform to all federal, state, and local regulations governing examining and licensing of pest control operators, performance of pest control, use of approved pest control chemicals and equipment, and proper disposal methods for all products/containers, which may be in effect for the area where the work under the contract will be performed. 2. Contractor ™s obligations to implement the IPM plan include, but are not limited to, the following tasks: (a) Inspect to determine pest management measures are appropriate and required, (b) Recommend and communicate to the COTR environmental sanitation practices that restrict or eliminate food, water or harborage for pests and recommend engineering practices that limit entry of pests, (c) Selection and utilization of non-chemical control methods, which eliminate, exclude or repel pests, i.e. insect electrocution devices, traps, caulking, air screens, etc., (d) Collection and disposal of all pests, (5) Selection and use of the most environmentally sound pesticide(s) to affect control when chemical control methods are necessary, (e) Control general structural arthropod pests (i.e., cockroaches, ants, carpet beetles, spiders, carpenter ants, carpenter bees, etc.), (f) Control flying insect pests (i.e., housefly, stable fly, blow flies, Asian beetle, etc.), (g) Control predatory pests (i.e., lice, bedbugs, fleas, mites, ticks, bees, wasps, mosquitoes, scorpions, etc.), (h) Control stored product pests (i.e., saw tooth grain beetle, red confused flour beetle, trogderma beetles, grain moths etc.), (i) Control mice and rats (i.e., house mouse, field mouse, roof rat, Norway rats, etc.), (j) Control pest birds (i.e., pigeons, sparrows, blackbirds, etc.), (k) Control other vertebrate pests (i.e., dogs, cats, bats, squirrels, gophers, moles, skunks, snakes, rabbits, raccoons, etc.), (l) Control wood destroying organisms (i.e., subterranean termites, dry wood termites, fungi, wood boring beetles, etc.), and (m) Control aquatic pests (i.e., mosquito larva/pupae, algae etc.) 3. Contractor will be required to perform inspections at monthly and quarterly frequencies. These frequencies are described in Appendix A. Upon inspection, should treatment be needed, contractor will treat appropriate areas. Additionally, the contractor will be required to treat all entryways during the fall season as certain pests are prone to finding access to the inside of the facility during this time. The frequency for this additional fall treatment will be at least once and on an ˜as needed ™ basis. NOTE: Fumigation treatments may require separate scheduling as approved by COTR for the convenience of the facility. E. CALL BACKS: EMERGENCY CALL BACK: The Contractor shall within two (2) hours after receipt of notification by the Contracting Officer or his/her designee, return a call and schedule the Integrated Pest Management (IPM) Services to correct the emergent condition. This call back service shall be accomplished at no additional charge to the Government. NON-EMERGENT CALLS: Contractor shall report within mutually agreed upon time with COTR for non-scheduled, non-emergent services with no additional charge to the Government. F. REPORTING FOR SCHEDULED SERVICES: Contractor shall report to Room 245, Building 1 on scheduled days for routine inspections of assigned areas. The Contractor will also obtain a security badge at this time. They will submit a written report of all activities following each visit, with the signature of the COTR or designee responsible. G. SAFETY FACTORS: All rodenticides or traps shall be placed only at times and in the areas approved by the COTR or his/her designee. Rodenticides, and/or traps shall be replaced as necessary and per manufacturer recommendations. No pest control material or trap shall be placed where it could be recovered by patients, nor shall any such material be allowed to contact food or cooking utensils. H. STORAGE OF PEST CONTROL MATERIALS: Storage of pest control materials or equipment at the SFVAHCS is prohibited without the written approval of the COTR. I. QUALITY ASSURANCE: All emergency complaints will be addressed and corrected within twenty four (24) hours. Non-emergent complaints will be addressed and corrected within time agreed upon by COTR and Contractor. Failure to meet prescribed timeframes shall not exceed 2 per month. J. REQUIRED CONTRACTOR REPORTING: 1. After each service (scheduled or non-scheduled) contractor personnel will submit documentation (as mentioned above) including but not limited to: (a) The name and address of the individual who performed the inspections, (b) The location, by building and room number where each inspection occurred, (c) The evaluation of each inspected area, (d) If needed, the areas which were treated and the pest or pests against which the treatment is intended to control, (e) The date and time of treatment, (f) The brand name of the pesticide applied, (g) The name of the pesticide manufacturer, or the federal environmental protection agency registration number of the pesticide, and (h) The rate of application or amount of the pesticide applied and the total area treated. 2. The contractor is responsible for completing and submitting all reports required or requested by Federal, State or Local ordinances which pertain to any duties contained in the contract. 3. The Contractor will furnish the COTR ™s office, prior to initial application, the trade names (if any) and the chemical names of all approved pesticides/chemicals along with appropriate antidote information and current Material Safety Data Sheet (MSDS). The Contractor shall supply this information in the same manner as new products are submitted for approval to the COTR. K. CONTRACTOR PERSONNEL BADGES AND PARKING: 1. The contractor shall provide the COTR with a list of contractor employees expected to enter the buildings to pick up confidential documents. While on VA premises, all contractor personnel shall comply with the rules, regulations, and procedures governing the conduct of personnel and the operation of the facility. 2. An access badge will be given to the contractor ™s employee upon entrance into VA buildings. The contractor employee must safeguard the access badge and immediately report any lost, stolen, or destroyed badges to the COTR. All contract personnel must properly display their access badges. Access badges must be worn at or above the waist (facing forward.). The contractor ™s employees must return the access badge(s) to the COTR or designee at the end of each pick up process. 3. The contractor shall be required to comply with all security policies/requirements of SFVAHCS. All security policies/requirements must be met and employees cleared prior to the contractor performing work under this contract. Employees that cannot meet the security and clearance requirements shall not be allowed to perform work under this contract. 4. It is the responsibility of the contractor ™s personnel to park in the appropriate designated parking areas. Parking information shall be coordinated with each facility COTR. 5. SFVAHCS does not validate or make reimbursement for parking violations of the contractor ™s personnel under any circumstance. L. INTERFERENCE TO NORMAL FUNCTION Contractor may be required to interrupt their work at anytime so as not to interfere with the normal functioning of the facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment and carts. In the event of an emergency, contractor services may be stopped and rescheduled at no additional cost to the government. Contractor personnel shall inform the COTR or the designee of the need to gain access to secured areas. If access is required to secure areas, prearranged scheduling will be made with COTR or designee.
