- Attachment II, Pricing Sheet dated 8-2-2011 Attachment I, Statement of Work dated 8-2-2011 HSCG44-11-R-PC2112 dated 08-2-2011 Amendment No. 0005; Amendment Language AMENDMENT Number 0005 To RFP HSCG44-11-R-PC2112 The purpose of this Amendment 0005 is to incorporate the following changes: A. Attachment I, Statement of Work: Attachment I, Statement of Work dated Jul 27, 2011 released as Amendment 1 on FBO dated July 27, 2011 is deleted in its entirety. Add the new Attachment I; Statement of Work dated 2 Aug 2011 provided with this amendment. The changes are annotated in the yellow highlighted areas. B. Attachment II, Pricing Sheet: Attachment II, Pricing Sheet dated July 27, 2011 released as Amendment 2 on FBO dated July 27, 2011 is deleted in its entirety. Add the new Attachment II; Pricing Sheet dated 2 Aug 2011 provided with this amendment. The changes are annotated in the yellow highlighted areas. C. HSCG44-11-R-PC2112: HSCG44-11-R-PC2112 dated Jul 27, 2011 released as Amendment 3 on FBO dated July 27, 2011 is deleted in its entirety.
