- Description
- The Centers for Disease Control and Prevention intends to issue a sole source firm fixed priced purchase order to SHAHDOKHT BOROUMAND located at: 416 Five Farms Lane Lutherville Timonium, MD 21093-2944. This is a special notice to determine the availability of potential vendors that can provide the necessary technical data for the NCCDPHP/DOH Division of Oral Health. The core tasks and minimum contractor requirements will include: The contractor shall complete these tasks. 1) Write a residency orientation handbook for new residents; 2) Develop the following residency documents: Residency Training Plan, Field experience report, Research protocol, Research report; 3)Coordinate and write periodic evaluations of resident's progress toward achieving training plan objectives; 4)Complete and submit annually specific information required by the Commission on Dental Accreditation; 5)Complete evaluation of applicants to residency program. The contractor must have the following qualifications: The vendor shall have a DMD or DDS with a Master's degree in public health and certificate of completion of dental public health residency accredited by CODA.The vendor shall be familiar with the CDC's Division of Oral Health (DOH), DOH resources and DPH programs. The vendor shall be experienced in the field of DPH, in all competency areas, addressing dental public health research, program administration, development and implementation, delivery of services and oral disease prevention and health promotion as evidenced by more than 5 years in the practice of dental public health. The vendor shall have experience in the development of written documents required for successful completion of 1) DPH residency programs by at least 5 residents and 2) comprehensive self-study reviews of at least two DPH residency programs for periodic CODA evaluations with the past three years and including a CODA review of CDC's DPH residency program. The proposed contract action is for services which the Government intends to solicit and negotiate with only one source under authority of FAR PART 6.302-1; 4202 Clinger-Cohen Act of 1996. Contractors interested in this requirement should submit a CAPABILITY STATEMENT THAT MUST DEMOSTRATE THE MINIMUM REQUIREMENTS OUTLINED ABOVE. Capability statements shall include the following information: company name, address, point of contact, phone/fax/email, and business size and status. Responses must be received by 4:00 p.m., Eastern Standard Time, August 16, 2011. Responses should be submitted to: Matthew Ellington 2920 Brandywine Road N.E. M/S E-01 Atlanta, GA 30341. This notice of intent is not a request for competitive proposals. However, the Government will consider all responses received before the closing of this notice. A determination by the Government not to compete this acquisition based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. This procurement is not set-aside for small business.
