Description
1. Introduction: This Statement of Work (SOW) sets forth basic requirements for Project NUEX 09-1408, Work Order# 81825, to repair HVAC Building 133. This design- build project will require performance at Luke Air Force Base, Arizona. This project will be an upgrade to the existing HVAC system in Bldg 133. 2. Scope: The existing HVAC system in Bldg 133 consists of Pumps, Chiller, Piping, Heating and Domestic Hot Water Boilers, and 2 (ea) Hot Water Storage Tanks, to include all accessories. The existing Cooling Tower, Plate and Frame Heat Exchanger, and Condenser pump were replaced in March 2011, and will be part of the project to re-connect. The existing mechanical system will require inspection to identify the extent of the current deficiencies (i.e., investigatory phase). Additionally, the construction and repairs to remedy existing deficiencies are requirements of this contract. It must be acknowledged by the contractor that the work scope includes complete review of the mechanical rooms, preliminary documents, validation of the preliminary work and standards referenced, and identification of deficiencies against present day standards for an Air Force and DoD facility. Under this contract, the contractor may be tasked with validating construction price/cost and scheduling. The contractor will complete project design to 100% and then go on to complete the construction. Construction may begin after approval of the 65% design. Contractor will deliver and warrant the equipment and materials replaced. The objective of this project is to deliver a complete and usable mechanical system, such that it supports the present day mission and meets present day standards. This project work will include all demolition, disposal, material, labor, and testing to deliver complete, operational, and usable systems. 2.1 Minimum Replacement/Repair items: Additional items may be required based on investigatory design. 2.2 The work will include the following, with a performance period of 180 calendar days after issuance of a written Notice to Proceed, but not necessarily be limited to: 2.2.1 Demolition: Provide the Government a list of equipment to be demo and disposed to determine what the Government will keep. All other materials will be the contractor's responsibility to disposed. Refrigerant from machinery shall be recovered and returned to the Government. 2.2.2 Replace existing chiller with appropriate type and size based on investigatory study (use screw type compressor for increased efficiency) 2.2.3 Remove and replace existing pumps for entire HVAC/R system 2.2.4 Provide VFDs where they are needed 2.2.5 Evaluate current electrical system and upgrade if this existing is not compatible with new HVAC components 2.2.6 Replace existing heating boiler with appropriate type and size based on investigatory study 2.2.7 Replace existing domestic hot water boiler with appropriate type and size based on investigatory study 2.2.8 Replace existing hot water storage tanks to include all accessories with appropriate type and size based on investigatory study 2.2.9 Replace current EMCS to improve efficiency 2.2.10 The contractor will provide complete test and balance of applicable equipment to serve as a basis for acceptance of the replaced/repaired installation 2.2.11 The contractor will provide training, and a complete operations and maintenance package, for the future operation of the upgraded HVAC system. 2.2.12 Possible Asbestos, contractor will have a thorough inspection performed and given to the government (56th CES/CEAN) prior to any work beginning. 2.3 Design Services: During the design phase of the project, the contractor will utilize a professional consultant, herein after referred to as the A/E. The A/E is responsible for the development of the design deliverables. Specifically, the effort will result in documents that are the basis for the design and construction efforts under this contract. This effort necessarily includes development of a design basis (to include applicable codes, standards, and guidance) and outline specifications. The contractor will complete the design documents incorporated by reference below. Investigatory phase: The contractor shall visit the job site to re-verify location, conditions, quantities and dimensions previously determined during the design portion of the project, as part of his construction proposal preparation. The contractor shall coordinate with the Contracting Officer prior to the visit to the job site. 2.4 Technical Services: The contractor is the "Architect of Record" and, as such, will sign and seal all completed designs. The contractor is accountable for all aspects of the final design under Federal, State and applicable local laws. For reference purposes, a "completed design" within the process includes, (1) the design which is developed within the design and review process, and (2) as-built drawings accurately reflecting any modifications to Bldg 133 HVAC system. Drawings and as-builts will be delivered as AutoCAD and PDF formats. The final records, test reports, to include O&M manuals, shall be in PDF formats, as finalized with personnel in Engineering at Luke Air Force Base. The contractor is responsible for construction quality on the project. The contractor is required to make their own field investigations to verify dimensions and other information shown on Government furnished reference drawings. Coordination between the (A-E) design disciplines in production of the contract drawings is the contractor's responsibility. 2.5 Summary Status Reports: The contractor shall provide weekly and monthly progress reports for this contract. All information will be maintained in an electronic database (compatible with Microsoft Office Professional, current version) to facilitate summary retrievals. Reports will be acceptable in contractor format. 2.6 Quality Control (QC) Oversight: The contractor shall review, plan and/or develop QC procedures for this contract, and submit the Quality Control Plan within 15 calendar days after award under this contract for government approval. 2.7 Hazardous Materials: Materials survey may include destructive testing as necessary to ensure adequate investigation for hazardous materials. Base Environmental personnel and standards will be consulted for approval in this regard. 2.8 Site Cleanup: The contractor shall be responsible for assuring that the existing adjacent site features are not damaged, and remain in their original condition at the completion of the contract work. If the contractor fails to clean up at the completion of the work, the Government may do so as required, and the cost thereof shall be charged to the contractor. All contractor-generated refuse and waste shall be hauled from the building construction area and deposited in his waste dumpster, or such waste shall be removed from the site daily to a disposal area to be selected by the contractor, and located outside the physical boundaries of the installation. This refuse and waste material shall be hauled in trucks with tight-fitting beds to prevent spillage on roadways. The construction site shall be kept neat, orderly, and safe for workmen at all times. 2.9 Design-Build: Prior to beginning onsite construction, the contractor will initiate the Construction Phase by completing project working drawings and the various plans and schedules, including a construction schedule and obtaining submittal approvals and permits. Working drawings and specifications shall comply with codes, laws, and the Luke General Specifications. Contractor will utilize the 48 Division specification and will provide a 65%, 95%, and 100% design. The Contractor shall conduct design review meetings (i.e., 65%, 95%, 100%), as well as progress meetings during the construction phase, with Contracting and Civil Engineering. The working drawings shall be submitted for review, as directed in this Statement of Work (SOW). The contractor shall be responsible for implementation, coordination, and execution of all regulatory reviews, ensure technical adequacy of the final design, and provide quality control of all phases of the contract. Further details regarding design requirements will be provided by the 56th CES/CEPM project manager. 2.10 Construction: Upon the approval of the Contracting Officer, the contractor may commence onsite construction activities. Construction shall be phased to minimize impact to building occupants. Construction schedule shall be coordinated with Facility Manager. Further details regarding construction management requirements, inspection and testing, construction facilities and temporary controls, environmental protection, quality control system, construction quality control, and project closeout will be provided by the 56th CES/CEPM project manager. 2.11 Security: The contractor shall obtain and monitor security badges used by their own staff during the duration of this contract, in accordance with Clause L-727 of this contract entitled Contractor Access to Air Force Installations. 2.12 On-Site Field Oversight: The contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure oversight services during the construction phase of this contract. 2.13 Safety Requirements: The contractor shall conduct operations in accordance with FAR Part 52.236-13 and Alternate 1. During the performance of the work within the requirements of this contract, and until final completion and acceptance thereof, the contractor shall exercise the utmost care to avoid accident or injury to persons or property. Contractor shall place and maintain all necessary barriers and safeguards, including watchmen, if necessary, about the work site for the prevention of accidents and shall take all precautions required to protect personnel and property to include the general public and properties adjacent to the work site. 2.14 Storage: The contractor shall conduct operations in accordance with FAR Part 52.236-10. The contractor shall confine construction operations at the site to areas permitted by law, ordinances, permits, and the contract documents and shall not encumber the site with any materials or equipment without prior written authorization. Subject to prior written approval of exact location, appearance, and color of each, the contractor may be permitted to set-up an office trailer/portable building, a similar job tool trailer, and a waste dumpster in a location designated and approved by the Contracting Officer. The contractor shall store other materials and equipment at the site designated by the Contracting Officer at the end of each workday. The contractor shall coordinate all operations with the written approval from the Contracting Officer before using any portion of the site. 2.15 Pre-Final Inspection: The contractor shall request a pre-final walkthrough inspection by the Government, with a minimum of two-weeks notice. The Government will provide the contractor with the pre-final inspection findings (i.e., punch list) in a pre-final inspection report. Contractor will have approximately one (1) week in which to correct all punch list deficiencies. 2.16 Final Inspection: Upon completion of all punch list deficiencies discovered during the pre-final inspection, the contractor shall request that the Government conduct a final inspection and publish the findings in a Final Inspection Report, in accordance with the format specified at the pre-construction conference. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final report. The final inspection report shall also certify that all items of the design have been implemented and that the construction is complete. The final inspection report shall also include a record of "signed and sealed" design drawings, and as-built drawings, and specifications, verifying that all development standards have been met. At the final inspection, the contractor shall present a completed DD Form 1354, Transfer and Acceptance of Real Property to the Base Civil Engineer (BCE) for signature and acceptance. 2.17 Delivery/Warranty: The contractor shall complete all inspection and commissioning requirements prior to final inspection. Following final inspection, the contractor shall deliver to the Government the as-built drawings in the format and media as required by this contract. Further details regarding project closeout will be provided by the 56th CES/CEPM project manager. The warranty shall be issued in accordance with FAR 52.246-21. Extended warranties offered by the contractor and its subcontractors or suppliers may be accepted by the Government at its discretion. 3. Contract Requirements: The contractor's administration shall include the following: 3.1 Full time site superintendence. 3.2 Submission for approval of: Construction schedule(s) (Microsoft Project), progress schedules, material cut sheets, shop drawings. 3.3 Submission for approval of: Dig permits, confined workspace permits (if applicable), utility outage requests, and invoices for progress payment(s). 3.4 General work products should be in Microsoft Office format. Drawings, O&M manuals and final submittals are discussed in Section 2.F. Technical Services. 3.5 After the issuance of this contract, the contractor shall attend all meetings and teleconferences at the location specified by the Contracting Officer's Technical Representative (COTR). 3.6 The contractor shall prepare and publish all meeting minutes following PCO approval, track all action items assigned, and shall document all items discussed at a meeting, as well as each meeting's attendees. 4. Applicable Documents: Base specific documents are identified below. In addition, the contractor shall comply with all applicable (1) Federal, state, local, DoD, Air Force, and agency laws; statutes, ordinances, instructions, manuals, handbooks, regulations, guidance, policy letters and rules (inclusive of changes and amendments), (2) Presidential Executive Orders in effect on the date of issuance of this contract. It must be acknowledged by the contractor that the work scope includes complete review of the mechanical systems and preliminary documents, validation of the development of the design basis, and provision of the final design basis against present day standards for an Air Force and DoD facility. 4.1 Documents hereby incorporated by reference: 4.1.1 UFC 1-200-07a Design Build Technical Requirements 4.1.2 UFC 1-300-09 DD 1354 Guide 5. Special Notification: The contractor shall immediately report to the Contracting Officer (CO), via telephone, any data or results generated during investigations pursuant to this contract which may indicate any potential imminent health risk to contracted or federal personnel, or to the public at large. Following this telephone notification, the contractor shall prepare a written notice with supporting documentation and deliver it to the COTR within three (3) working days. The contractor shall provide written notification in the form of a signed letter scanned into Adobe Acrobat Portable Document Format (*.pdf) and send it to the Contracting Officer via email. Upon request of the COTR, the contractor shall provide pertinent raw laboratory data within three (3) weeks of the telephone notification. 6.
