Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This acquisition will be conducted in accordance with FAR 13, Simplified Acquisition Procedures. This solicitation document incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53 dated 4 August 2011. The solicitation number for this requirement is W91QF411-T-0003. This solicitation is issued as a request for quotes (RFQ). This acquisition is unrestricted. The associated North American Industry Classification System (NAICS) code for this procurement is 515210, with a Standard Industry Classification (SIC) code of4841. The Small Business size standard for this requirement is $15.0 million dollars. Fort Leavenworth, KS has a requirement for satellite television service at the two facilities associated with the Military Correctional Complex. Included the in the requirement are specified television stations, warranties for all equipment, and on-site and phone technical support. The quotes will be evaluated in accordance with FAR Part 13. An award will be made to the lowest priced offeror that is technically acceptable. As part of price evaluation, the Government will evaluate its option to extend services (see FAR Clause 52.217-8) by adding six (6) months of the offeror's final price to the offeror's total price. The requirements consist of the following: CLIN 0001: Satellite service for 1 year CLIN 0002: Contractor Manpower Reporting (CMR) for Base Year CLIN 003: Option for additional year of Satellite service CLIN 004: Contractor Manpower Reporting (CMR) for 1st Option Year CLIN 005: Option for additional year of Satellite service CLIN 0006: Contractor Manpower Reporting (CMR) for 2nd Option Year Quotes should include detailed pricing for all Clins above, a list of channels that will be provided, and a description of the technical support that will be provided. Past performance information of recent and relevant project should be submitted. All information should be based on the Performance Work Statement blow: 1. PERFORMANCE WORK STATEMENT: 1.1 Non-Personal Service: The Contractor shall provide the necessary hardware, parts, labor, materials, and programming to provide Satellite Television Service to two locations at the Military Correctional Complex at Fort Leavenworth, Kansas, consisting of the United States Disciplinary Barracks, 1301 North Warehouse Road, Fort Leavenworth, Kansas 66027; and the Joint Regional Correctional Facility, 830 Sabalu Road, Fort Leavenworth, Kansas 66027. 1.2 The Contractor will provide service for thirty-four (34) public connections at the United States Disciplinary Barracks, service for twenty-seven (27) public connections at the Joint Regional Correctional Facility. The Contractor will provide the capability to activate two 16-channel head-end configurations for specific connections designated by the customer to provide, as a minimum, the following 16 channels to each prison: CNN, TNT, USA, ABC, CBS, FOX, NBC, AMC, BET, SCYFY, A&E, FX, COMEDY CENTRAL, ESPN, ESPN2, and UNIVISION. 1.3 The Contractor shall also supply 3 months' warranties on dish alignment and regular warranties on equipment. 1.4 The Contractor will activate all receivers with the Customer Points of Contact, and ensure all connections are receiving proper signals. 1.5 Period of Performance: The period of performance will consist of one basic year of service with two (2) one (1) year option periods. 1.6 Contractor Customer Service: The Contractor will provide 24hr telephone technical/customer service support and on-site technical/customer service support within 12 hours. 1.7 Government Furnished Equipment: The Government will provide coax cable to each public connection and hookups in a central location. INFORMATION TO OFFERORS: 2.1 The Military Corrections Complex (MCC) at Fort Leavenworth, Kansas consists of the United States Disciplinary Barracks (USDB), and the Joint Regional Correctional Facility (JRCF). Hereafter, the term "MCC" shall refer to the USDB or the JRCF, as appropriate. Access to and work inside the MCC is subject to the below restrictions: 2.2 The USDB is a Level III maximum security military prison. The JRCF is a Level II medium security prison. The security and safety of all concerned parties shall be paramount. 2.3 Each prison has its own Physical Security Officer. Hereafter the term "Physical Security Officer" shall refer to either the USDB or JRCF Physical Security Officer, as appropriate. 2.4 Contractors shall submit an access roster to the Physical Security Officer of the prison(s) into which they require entry. The access roster shall be a list of all Contractor employees and Contractor vehicles, to include types of vehicles, color and license plates. Contractors shall submit access rosters at least ten working days before beginning work to allow the Physical Security Officer time to conduct security investigations for Contractor employees to be permitted on MCC property. Contractors will notify the Physical Security Officer in writing a minimum of ten working days in advance to add employees or equipment to the work force. Contractors shall immediately notify the Physical Security Officer in writing should a Contract employee(s) need to be removed from the work force. The Physical Security Officer reserves the right to deny Contract employees access to the MCC, should the results of the security investigation(s) determine them a security risk. This will not negate the contractor's responsibility to provide an adequate number of employees who meet security requirements, to perform services required under the contract. 2.4 All Contractor employees, vehicles and materials shall be subject to inspection and search upon entry into and exit from the MCC. Except for emergency vehicles (fire trucks and ambulances), Contractor employees and vehicles shall be given first priority in entering and departing the MCC. All Contractor employees shall obey special handling or parking instructions for equipment that could be used to breach security when idle. 2.5 All Contractor employees shall read and comply with USDB Regulation 190-3, Standards of Conduct in its entirety, and sign a Statement of Understanding (SOU) prior to beginning work. Contractors will attach signed SOU's to their access rosters upon submittal to the Physical Security Officer in order to process individual contractor employees for clearance. Contractor employees will enter and exit the MCC through the West Service Gates for each prison. Contractor Employees will sign for Contractor badges when entering and leaving West Service Gates. After West Gate Corrections Specialists identify and clear them, Contract employees will sign in and sign out on the West Gate register to enter and depart the MCC. Contract employees shall wear their Contractor badge on the outer garment at all times. Contract Employees who lose a badge must immediately report the loss to the Physical Security Officer. 2.6 When MCC Corrections Specialists search Contractor vehicles entering or leaving the MCC, all Contractor employees shall exit their vehicle, shut the engine off, and open all doors and compartments. 2.7 Contractor vehicles will not be permitted to enter or depart the West Service Gates between 1100 and 1230, except those deemed necessary to complete ongoing work. 2.8 Privately owned vehicles will not be allowed in the MCC. 2.9 Contractor employees will not leave vehicle keys in the ignition switch at any time the vehicle is unoccupied. Contractor employees will lock all unattended Contractor vehicles. Contractor employees will render inoperable all idle contractor vehicles, or equipment without key ignitions. 2.10 Once inside the walls of the MCC, Contractor employees shall travel only from the West Service Gate to the work site. All Contractor employees will remain at designated work sites. Corrections Specialists will escort Contractor employees who require access to additional work sites. 2.11 Contract employees are not permitted to talk with prisoners, except as actual work may require. Contractor employees shall not give or accept any items from prisoners at any time. If there are problems with prisoners, Contractor employees will notify a Corrections Specialist or the Physical Security Officer. 2.12 The following items are contraband, and Contractor employees will not bring them through the West Service Gates of the MCC: All tobacco products, to include cigarettes, cigars, pipe tobacco, or smokeless tobacco. Guns and firearms of any type Ammunition/Explosives Knives (except those required as part of the tools used for the job) Hazardous and poisonous chemicals and gasses Narcotics and drugs, to include marijuana in any form Intoxicant, alcoholic beverages Unattended currency Razor blades (except those included in toolkits) Obscene printed material (i.e. nudity emphasizing a state of sexual arousal, graphics of body orifices and portrayal of explicit sexual acts) Photographic devices/equipment, audio or visual recording devices Extra clothing not worn on the job 2.13 Contractor employees will secure all tools and equipment under lock and key at all times, unless in actual use. 2.14 The Commandant, USDB, or the Commander, JRCF may remove and permanently bar Contractor employees from the MCC for actions that the Commandant, USDB, or the Commander, JRCF deem may threaten or compromise the security of the MCC. Notwithstanding the removal and/or barring of such employees, the Contractor shall continue to perform, in full, all aspects of the job in accordance with the provisions of the contract. 2.15 The Commandant, USDB or the Commander, JRCF may grant exceptions to the above instructions on a case-by-case basis. Contractors shall submit exception requests to specific items above to the Contracting Officer, Fort Leavenworth, KS three (3) weeks prior to the proposal response date. If an exception is approved, all offerors in receipt of the Request for Proposal will be so notified before the proposal response date. Exceptions to these instructions after work commences will generally not be granted. The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. b. The contractor is required to completely fill in all the information in the format using the following web address https://cmra.army.mil. c. The required information includes: (1) Contracting Office, Contracting Officer, and Contracting Officers Technical Representative; (2) Contract number, including task and delivery order number, (3) Beginning and ending dates covered by reporting period, (4) Contractor name, address, phone number, E-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including subcontractors); (6) Estimated direct labor dollars paid this reporting period (including subcontractors); (7) Total payments (including subcontractors); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each subcontractor if different); (9) Estimated data collection cost; (10) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (11) Locations where contractor and subcontractors perform the work (specified by zip code in the United States and nearest city, country, when in an overseas location, using standardized nomenclature provided on web site); (12) Presence of deployment or contingency contract language; and (13) Number of contractor and subcontractor employees deployed in theater this reporting period (by country). d. As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. e. Reporting period will be the period of performance not to exceed 12 months ending 30 September of each government fiscal year and must be reported by 31 October of each calendar year. f. Contractors may use a direct extensible Markup Language (XML) data transfer to the database server or fill in the Fields on the web site. The XML direct transfer is a format for transferring files from a contractor's systems to the secure web site without the need for separate data entries for each required data element at the web site. The specific formats for the XML direct transfer may be downloaded from the web site.
