- Attached: Response to Questions. The due date for quotes has been extended from Monday, August 8, 2011 at 10:00 AM EST to Tuesday, August 9, 2011 at 5:00 PM EST. The Uniformed Services University of the Health Sciences (USUHS), located in Bethesda, MD has a requirement for a service maintenance agreement of the following laboratory (histology) equipment by Leica Microsystems, Inc.: 1) EM UC62io Ultracut; 2) CM3050 S Configured Cryostat; 3) Autostainer XL 100, and 4) CV5030 Coverslipper. The service agreement shall provide preventive maintenance and repair of the stated laboratory equipment, which is utilized for scientific research by faculty and graduate students at USUHS. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation incorporates all relevant provisions and clauses in effect through the Federal Acquisition Circular (FAC) 05-53. The solicitation number is HU0001-11-Q-0056. This request for quotes (RFQ) is issued in accordance with FAR Part 12 - Acquisition of Commercial Items and FAR Part 13 - Simplified Acquisition Procedures. It is the responsibility of the contractor to monitor FedBizOpps for any amendments to the solicitation. An option year contract is anticipated for this award. The government will award a single, firm fixed price (FFP) purchase order(s) resulting from this solicitation to the responsible offeror(s) whose quote meets the requirements of the solicitation. A single firm fixed priced award will be made based on best value to the government. Best value determination will be based on price, technical and past performance. USUHS reserves the right to award without discussions. This solicitation is set aside for small businesses and the applicable NAICS code for this requirement is 811219 - Other Electronic and Precision Equipment Repair and Maintenance. The small business size standard is $19.0 MIL. Statement of Work: See attached document. Quote Format: Offeror must provide the company DUNS with quote. The contractor shall provide pricing for one (1) base year (12 months) with one (1) one-year option for the EM UC62i Ultracut, and one (1) base period (12 months) with four (4) one-year options for the remaining three (3) units identified on the attached pricing Excel spreadsheet. Delivery: The place of performance is. A detailed address will be included in award. Evaluation Factors: 1. Price - Offerors must provide a price for each listed item on the attached spreadsheet for the base year and each option year requested. Offerors who do not provide base and option year (I - IV) pricing for each item, as requested, will not be considered for award. 2. Technical/Corporate Experience - Offerors must provide a narrative of their corporate experience in providing the requested services. In addition, offerors must provide details of what is covered in the service agreement. Details should include at a minimum: address emergency repair service; hours of operation for emergencies and non-emergencies; cost of labor, travel and replacement parts; number of PM visits per year. 3. Past Performance - Offerors must provide contact information (Name, Title, Company, Phone Number, Email) on two (2) contracts for similar service within the past three (3) years. A brief description of the services provided, dollar amount, performance period and contract/order number must be included. Favorable past performance on similar/relevant orders will be considered more advantageous than favorable past performance on less relevant past experience. USU reserves the right to use any other means to evaluate past performance. The performance period for the base year is expected to start five days after receipt of order. Please see attached for applicable provisions and clauses. Offeror must complete Provision 52.222-48 attached (page 7). To be eligible for award, ALL offerors from MUST be registered in CCR (Central Contractor Registration) at https://www.bpn.gov/CCRSearch/Search.aspx, and ORCA (Online Representations and Certifications Application) at https://orca.bpn.gov/login.aspx. All questions must be directed by email no later than 2:00 PM EST on Tuesday, August 2, 2011. Questions will be addressed in the amendment to the RFQ; no questions will be answered over the phone. Quotes are due by Tuesday, August 9, 2011 by 5:00 PM EST. Quotes must be emailed to Benjamin.Kacher@usuhs.mil and Clarinda.Washington@usuhs.mil. No other methods of submission will be accepted.
