Description
1 This is a combined synopsis/solicitation for commercial services prepared IAW the format in FAR Subpart 8.405, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. The solicitation W912L6-11-T-0008 is issued as a Request for Proposals. *NOTE* Solicitation is identified as W912L6-11-T-0005" on the attached SF1449. Please submit responses and questions regarding this solicitation referencing "W912L6-11-T-0008". The solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 200 5-21, dated 11/7/07. No DPAS rating is applicable. 1.2 This Solicitation and any Amendments can be downloaded and printed from the General Services Administration (GSA) at https://acquisition.army.mil. Interested parties may register for automatic updates, and it is incumbent upon the interested parties to review the website frequently for updates to the documents. 1.3 The associated North American Industry Classification Systems (NAICS) Code is 621999 (Health Screen Services). This procurement has not been set aside or restricted to small business and is being competed on an unrestricted basis. 1.4As a condition of award, the successful Contractor must include the foregoing NAICS Code, or a comparable NAICS code that adequately describes the services required by the resulting contract, on its CCR registration. 1.5 The purpose of the Occupational Medical Surveillance Examination Program is to provide job-related Medical Surveillance Examinations and Occupational Health Services to the Oklahoma National Guard (OKNG) full-time support (FTS) and/or temporary personnel who are or potentially could be exposed to health hazards in the work environment. The examination shall include personnel assigned to positions requiring specific standards of physical fitness. Personnel not included under these examinations are Oklahoma State Employees, civilian contractors, and the OKNG part-time soldiers, known as M-Day soldiers. 1.6 Government reserves the right to issue one or more IDIQ's regarding the same or similar scope of work.
