Bid Sheet (base year + four option years) Request for Quotation This acquisition shall be 100% Competitive Small Business Set Aside - Request for Quotation (RFQ). All Small Business responsible sources may submit a quote. This procurement will be conducted under NAICS Code 562111 with a small business threshold of $12.5 Million. Anticipated contract length is five (5) years total (Base Year plus four (4) Option Years.) Estimated Cost Range is between $250,000 and $750,000 for all five years. The Contractor shall furnish all labor, parts, materials, and equipment necessary to fulfill contract obligations, in addition to providing regular weekly and/or biweekly services for each dumpster. The work required under this contract provides for a comprehensive service contract for Lake Mendocino and its recreation areas. PERIOD OF PERFORMANCE: Base Year (BY): 01 October 2011 through 30 September 2012. Option Year 1 (OY1): 01 October 2012 through 30 September 2013. Option Year 2 (OY2): 01 October 2013 through 30 September 2014. Option Year 3 (OY3): 01 October 2014 through 30 September 2015 Option Year 4 (OY4): 01 October 2015 through 30 September 2016.
