Description
SOLICITATION NOTICE Preventive Maintenance and Repair of Hemodialysis Equipment VA Caribbean Healthcare System, San Juan, PR The Department of Veterans Affairs (DVA), VA Caribbean Healthcare System (VACHS), Network Contracting Activity-8 (NCA-8), Tres Rios Building, San Patricio, 27 Ave. J. Gonzalez Giusti Suite 600, Guaynabo, PR 00968 intend to solicit offers on a Cascading Small Business Set Aside-Lowest Price Technically Acceptable (LPTA): Small Business set aside (including in order of preference: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), and Small Businesses. The procurement seeks for Preventive Mainterance and Repair on Hemodialysis Equipment services under applicable North American Industry Classification System (NAICS) code number 811219. The DVA intends a contract with a base period from date of award to September 30, 2012. It will include four (4) years period options to be exercised at the discretion of the Government. The qualified Small Business firm shall provide the maintenance and repair of the hemodialysis equipment and systems hardware and software as specified in the Schedule of Supplies. QUESTIONS: shall be submitted in writing via facsimile or e-mail attention to the Contracting Officer no later than 3 (August 16, 2011) days after the solicitation is posted at Fedbizopps (FBO) web site: http://www.fedbizopps.gov/. VERBAL OR TELEPHONE INQUIRIES WILL NOT BE ACCEPTED. All responses will be answered and incorporated into a written amendment to the solicitation, for the benefit of all interested offeror's. Solicitation No. VA-248-11-RP-0896. FAXED PROPOSALS WILL NOT BE ACCEPTED. Offerors shall go to FBO web site to download subject solicitation package, including any related attachments and/or amendments, when available. In case the solicitation due date is extended, all interested offeror's shall be responsible to monitor the changes posted at FBO web site, prior the solicitation due date. Each offeror shall acknowledge receipt of each amendment, submitting each amendment attention to the Contracting Officer (CO) at Department of Veterans Affairs, VA Caribbean Healthcare System, NCA-8, Tres Rios Building - San Patricio, #27 Ave. J. Gonzalez Giusti Suite 600, Guaynabo, PR 00968 by the solicitation due date: August 22, 2011 at 9:00 am. The Government reserves the right to award without discussions; therefore, the Small Business offeror shall submit the most favorable terms and technical information with their offer; including their price proposal considering it will be awarded as LPTA-Refer to FAR 15.101-2 for information on this source selection process. All interested Small Business offerors shall be registered in the Central Contractor Registration (CCR) database before submitting their offer, and ORCA registration shall be in place prior to award. Contractors are responsible for the accuracy of information added to CCR. To be registered, you must go to http://www.ccr.gov/. Current contract information may be requested and disclosed under Freedom of Information Act (FOIA), in accordance with 38 Code of Federal Regulations (CFR) 1.553(b). FOIA request shall be addressed attention to Ms. Rosayma Lopez, Privacy Officer, Department of Veterans Affairs, VA Caribbean Healthcare System, 10 Calle Casia, San Juan, PR 00921-3201. All requests shall be in writing by any firm or person, including Requestor's signature, Sender address and print name. Copy of FOIA request may be faxed to the CO for tracking purposes only, but all follow up shall be done with the Privacy Officer. Each request shall reasonably describe the records, so that they may be located with a reasonable amount of effort. Also each FOIA Requestor shall states their willingness to pay applicable fees or provide justification to support a fee waiver. A day time telephone number is required. All envelopes and correspondence must state Freedom of Information Act (FOIA) Request. Discussions are not allowed, all questions shall be send attention to the Contracting Officer (CO) for the benefit of all interested parties.
