- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation RFQ number VA-246-11-RQ-0435 is issued as a request for quote (RFQ). No telephone requests will be accepted. Only written requests received directly from the requester are acceptable. It is the Offerors responsibility to ensure that the quote is received. The associated North American Industry Classification System (NAICS) code for this procurement is 423450, with a small business size standard of 100. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON THEIR QUOTE. This action is unrestricted. If offeror has a FSS/GSA number, please include it on your offer. The Department of Veterans Affairs, VISN 6, Salisbury VA Medical Center has a requirement for the following: 614-28MIOA Connect Motorized Recliners12 Each 400CWFCustom stain to match Charge wood stain to match customer provided sample 1 Each Item numbers referenced above are IOA. This is a BRAND NAME OR EQUAL requirement. If providing a quote on an "equal" product- it will be evaluated on the following requirements. -Must be able to elevate a patient from seat height to exam table height (comparable to 19" to 32") -Motorized chair operates from the recliner seat with a handheld paddle. -Allows patient to elevate footrest, heat the seat & lumbar, and elevate or recline the back to a flat or fully reclined position, a sleeper. -The footrest and back have infinite positions - can be stopped and it will lock or hold position. -Motorized chair is able to move while fully supporting patient to reach a comfortable position. -Motorized chair is able to stay in a semi-reclined position to offer more support for those patients that cannot lay flat. "Or equal" offers must include a full product description and address the requirements above. Delivery shall occur within 120 calendar days of contract award, shall be delivered to the VA Medical Center, 1601 Brenner Avenue, Salisbury, NC 28144. The contractor must indicate if there are freight charges. Quotations shall include the RFQ number, the proposed unit price, the proposed extended price, any prompt payment discount terms, any shipping charges, company name, and point of contact, address, phone number, fax number, tax identification number, DUN number, and size status (i.e., small business, veteran owned small business, disadvantaged business, woman owned business, etc). Interested persons may identify their interest and submit a quote. The Government shall consider all proposals received by 11:00 AM Eastern Standard Time, August 18, 2011.
