- This is a combined solicitation/synopsis for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request For Quotation (RFQ), solicitation number FA4608-11-Q-S953, in accordance with FAR Parts 12 and 13. The North American Industry Classification System (NAICS) number is 813110, and the business size is $7,000,000.00. The proposed acquisition is set-aside for small businesses This requirement is for a base year and four options and the service being procured is as follows: Protestant Praise Music Director - See Attached Statement of Work Offeror Must submit Quote, Resume with References and Representations and Certifications. The evaluation criteria for the interview questioning process is either acceptable or unacceptable answers. Interview questions are attached. **Interviews will be conducted from 30 August 2011 - 1 September 2011. Interviews will be either face to face or through teleconference. Face to face interview is preferred, but not mandatory. Face to face interviews will be held at Chapel 1 located on Barksdale AFB, between the hours of 8:00 a.m. and 4:00 p.m. Other written material may be submitted to support oral presentation, but no media will be accepted. In addition, the interview will be a maximum of one hour. Technical Acceptability includes the following: ______ Have a minimum of three years experience in a music related field such as instrument, voice, and /or conducting; two years of which must be in the church setting Formal education or equivalent experience in music and/or certification by appropriate professional organizations is desired (identify in resume). ______ Have a minimum of three years experience in leading/directing congregational music, choirs, praise teams, and other special music groups (identify in resume). ______ All applicants must provide acceptable answers to all interview questions Offerors must comply with all instructions contained in FAR 52.212-1, instructions to offerors Commercial Items, applies to this acquisition and the following addenda applies. Offerors shall submit their resume and quote on letterhead stationery annotating Cage Code Number, DUNS Number and Tax Identification Number. THE GOVERNMENT WILL NOT ACCEPT INCOMPLETE PROPOSALS. Offers will be evaluated on technically acceptable, price and in compliance with all directions in this solicitation and attached statement of work. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with every requirement, that offer will not be technically acceptable, and therefore will not be considered.
