Description
This is a pre-solicitation notice for Halfway House Services in accordance with FAR Part 5. The Department of Veterans Affairs, Birmingham Veteran Affairs Medical Center (BVAMC), 700 S. 19th Street, Birmingham, AL 35233, is seeking sources for a potential contract to provide Halfway House Services for eligible veterans referred from the Birmingham VA Medical Center. The contractor's facility must be within 15 miles of the BVAMC. The contractor's facility must also be located within.50 miles from access to the Public Transit System. The intended contract is anticipated as a one base year with two (2) one year option periods. Note that this requirement mandates that a contractor must be incompliance with the Life/Safety checklist at the time of the submission of the proposal or the proposal will be considered non responsive. Orders will be placed at time of award (or at time of option(s) exercised if applicable) for the estimated quantity for that period of performance. BASE YEAR: 26 Sept 2011 through 25 Sept 2012 CLIN 0001 Halfway House Services DESCRIPTION: Per Diem Rate Per Patient based on a 24 hours service EST QTY: 15 Patients/Beds Per Day = 5475 Patients/Beds per Year EA OPTION YEAR ONE: 26 Sept 2012 through 25 Sept 2013 CLIN 1001 Halfway House Services DESCRIPTION: Per Diem Rate Per Patient based on a 24 hours service EST QTY: 15 Patients/Beds Per Day = 5475 Patients/Beds per Year EA OPTION YEAR TWO: 26 Sept 2013 through 25 Sept 2014 CLIN 2001 Halfway House Services DESCRIPTION: Per Diem Rate Per Patient based on a 24 hours service EST QTY: 15 Patients/Beds Per Day = 5475 Patients/Beds per Year EA The solicitation (Request for Proposal) will be posted on 7 Sept 2011 August 2011 via www.fbo.gov under VA-247-11-RP-0327. Contractor services will be furnished to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs (VA) and are homeless with substance abuse and/or other mental health diagnosis. Negative drug screens will not be a requirement for admission. It is understood that the type of patients to be cared for under this contract will require care and services above the level of room and board. The per diem rate established will be an all inclusive rate. Care will include, but not be limited to: 1.Structured group activities, including physical activities as appropriate. 2.Instruction in and assistance with health and personal hygiene. 3.Monitoring of medications, including locked drawers for storage of medication. 4.Supportive social services, in collaboration with the HCHV/HUD-VASH program staff, or other contract resources. 5.Individual case management, including professional counseling on self care skills, adaptive coping skills as appropriate, vocational rehabilitation referrals. 6.Assistance in learning and development of responsible living patterns to achieve a more adaptive level of psychosocial functioning upgraded social skills and improved personal relationships. 7.Support for an alcohol/drug abuse free lifestyle by maintaining a drug and alcohol free environment. 8.Routine and as needed/indicated breathalyzer and urine drug screening. 9.Assistance in learning, testing and internalizing knowledge of the illness/recovery process for homelessness. 10.Employment placement and job training. 11.Housing services to include permanent affordable placement. 12.No pornographic material allowed on the premises. This applies to staff and Veterans. 13.Employees should not fraternize with Veterans. The anticipated award date: 26 September 2011.
