Amendment 4 is issued to provide answers to the following questions and provide additional information as follows: 1.QUESTION: What time on the 25th is the bid due? RESPONSE: As stated in Solicitation # W91248-11-T-0027, quotations are due to this office by 10:00 A.M. (CST) 25 August 2011. 2.QUESTION: Is there a formal bid form? RESPONSE: Reference Solicitation # W91248-11-T-0027 and subsequent amendments. Pricing shall be submitted on the bid schedule included in this solicitation. Refer to Amendment 2, posted 8 August 2011, for the revised bid schedule. 3.QUESTION: Where do we deliver the bid? RESPONSE: As stated in Solicitation # W91248-11-T-0027, facsimile quotations are acceptable at 270-798-7820; mailing address is Mission and Installation Contracting Command-Fort Campbell, 2176 13-1/2 St, Fort Campbell, Kentucky 42223-5355. Email submissions shall include the following individuals: ashley.witmer@us.army.mil, jeanne.shykes@us.army. mil, and lilia.wood@us.army.mil. 4.QUESTION: When are the questionnaires due in your office? RESPONSE: Past Performance Questionnaires are due to this office by the closing date and time of this solicitation. Refer to Solicitation # W91248-11-T-0027 and Question 1 above for the closing date and time. 5.QUESTION: Is there currently a contract for this requirement now and if who is it? RESPONSE: This question was previously addressed. Refer to Amendment 2 posted 12 August 2011. 6.QUESTION: Is this a bonded bid? RESPONSE: No. 7.All email responses to this solicitation shall include the following individuals: ashley.witmer@us.army.mil, jeanne.shykes@us.army. mil, and lilia.wood@us.army.mil. 8.All questions/requests for clarification must be received by 4:00 P.M. (CST), 22 August 2011. 9.The following FAR Clause is hereby incorporated into this requirement: FAR Clause 52.228-5, Insurance -- Work on a Government Installation (Jan 1997) (a) The Contractor shall, at its own expense, provide and maintain during the entire performance of this contract, at least the kinds and minimum amounts of insurance required in the Schedule or elsewhere in the contract. (b) Before commencing work under this contract, the Contractor shall notify the Contracting Officer in writing that the required insurance has been obtained. The policies evidencing required insurance shall contain an endorsement to the effect that any cancellation or any material change adversely affecting the Government's interest shall not be effective -- (1) For such period as the laws of the State in which this contract is to be performed prescribe; or (2) Until 30 days after the insurer or the Contractor gives written notice to the Contracting Officer, whichever period is longer. (c) The Contractor shall insert the substance of this clause, including this paragraph (c), in subcontracts under this contract that require work on a Government installation and shall require subcontractors to provide and maintain the insurance required in the Schedule or elsewhere in the contract. The Contractor shall maintain a copy of all subcontractors' proofs of required insurance, and shall make copies available to the Contracting Officer upon request. (End of Clause) 10.The following information is incorporated: SCHEDULE OF INSURANCE: a. The Contractor shall, at its own expense, provide and maintain during the entire performance period of this contract at least the kinds and minimum amounts of insurance required below: Workmen's Compensation and Employer's Liability Insurance $100,000.00 General Liability Insurance for Bodily Injury Liability: Minimum Per Occurrence $500,000.00 Automobile Liability Insurance: Minimum Per Person $200,000.00 Minimum Per Occurrence for Bodily Injury $500,000.00 Minimum Per Occurrence for Property Damage $20,000.00 b. Before commencing work under this contract, the Contractor shall certify to the Contracting Officer in writing that the required insurance has been obtained. The policies evidencing required insurance shall contain an endorsement to the effect that any cancellation or any material change adversely affecting the Government's interest shall not be effective (1) for such period as the laws of the State in which this contract is to be performed or prescribed (2) until 30 days after the insurer or the Contractor give written notice to the Contracting Officer, whichever period is longer.
