Description: This is a sources sought notice only; it is not a request for quotations or offers. The Tennessee Valley Healthcare System is conducting market research to identify potential sources for a contract to provide offsite Cardio Thoracic Surgery for the VAMC Lexington, KY. Services shall be available from the Contractor upon notification by the Veterans Administration Medical Center (VAMC). Routine scheduling will be available as deemed necessary by the consulting Cardiothoracic Service. It is mandatory that emergency service shall be available within one (1) hour of notification seven (7) days per week, twenty-four (24) hours per day including all holidays. NAICS Code 621111 applies with a size standard of $10 M. Interested parties should respond to the Contract Specialist, Tina Smith by e-mail at Christina.Smith7@va.gov. All interested parties should include the following information: 1. A company profile, including name of business, DUNS number, and other pertinent information 2. A description of capabilities. 3. Past performance data for similar size facilities, multiple locations which demonstrate your capability to satisfactorily execute a contract to provide primary medical care services as described in this notice. Data shall include customer name, address, contract number, and point of contact information (name, phone number, and e-mail address.) 4. The business size and business socioeconomic category; such as Service-Disabled Veteran Owned Small Business, Veteran-Owned Small Business, Hub Zone Small Business, etcĂ ‚¬ ¦ Please note, all SDVOSB ™s and VOSB ™s must be verified by the office of Small Disadvantaged Business Utilization ™s Center for Veterans Enterprise (CVE) prior to receiving any award. Interested parties should respond not later than August 24, 2011 at 2:00 P.M. Central Time. This sources sought notice is to assist the Tennessee Valley Healthcare System in identifying an appropriate acquisition strategy only. Written communication received will be considered solely for the purpose of determining current market capabilities for the services identified within this notice. A solicitation is not currently available. If a solicitation is issued, it will be announced and posted on this website at a later date. This notice does not commit the government to contract for any supply or service. The government will not pay for any information or administrative cost incurred in response to this source sought notice. Do not contact medical center staff in regards to this requirement, as they are not authorized discuss this matters related to this procurement action. No phone calls will be accepted.
