- Past Performance Questionnaire Performance Work Statement (PWS) Aug 10, 2011 RFP DOL111RP21334 - "Women Veterans Who Are Homeless or at Risk of Becoming Homeless" 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. The solicitation is issued as a Request for Proposal (RFP). 3. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 05-52, dated May 31, 2011. This acquisition is set-aside for Small Businesses and will be total Small Business set-aside. The associated NAICS: 541611 -- Administrative Management & General Management Consulting, which has a small business size standard of $7.0M. 4. The contract type will be Firm Fixed Price (FFP). 5. The period of performance is anticipated to be: Base Period (6 months): September 30, 2011-March 29, 2012 Option Period 1 (6 months): Mar 30, 2012 - September 29, 2012 6. Offerors are reminded that failure to comply with the Performance Work Statement (PWS) (RFP Attachment 1), may result in the termination for cause of the contract pursuant to FAR 52.212-4(m). 7. Description of the Requirements - See Attachment #1 for the Performance Work Statement (PWS). 8. Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offerors-Commercial Items, with the following Addendum: A. Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation. B. No alternate offers will be accepted. C. A single contract award will made as a result of this RFP. D. Offers shall include completed Representations and Certifications, including - Copy of completed electronic annual representation and certification at the Online Representations and Certifications Application (ORCA) database (http://orca.bpn.gov) - 52.212-3, Offeror Representations and Certifications -- Commercial Items (May 2011), Alternate I (Apr 2011) - - FAR 52.219-1, Small Business Program Representations (APR 2011) - FAR 52.219-28 - Post-Award Small Business Program Re-representation (Apr 2009). E. Offerors shall submit proposals in electronic format as attachments via email files and shall not contain classified data or hyperlinks or self extracting files. Only MS Word, MS EXCEL and Adobe formats for submission of proposals will be accepted. Other formats will be considered nonresponsive and rejected, if submitted in any other formats, including:.b64,.bat,.bhx,.ceo,.ce0,.cpl,.dbx,.dll,.dot,.eml,.exe,.hqx,.lnk,.max,.mim,.nch,.ocx,.pi,.pif,.scr,.sct,.uue,.uu,.vbe,.vbs,.wsc,.wsf,.wsh,.xxe, and.zip. F. Offerors shall submit a Technical Proposal (Volume I) and Price Proposal (Volume II). Technical Proposals shall address the non-price evaluation criteria under the Technical Approach Factor and Past Performance Factor, and shall not contain any pricing information. The Technical Proposal (Volume I) shall not exceed 25 pages. Information which exceeds the 25-page limit will not be considered. One (1) page is equal to one side of a sheet of paper. Two (2) pages are equal to the front and back sides of a sheet of paper. Offerors shall use 8-1/2 x 11" paper; one (1) inch margins; and, Font size no smaller than size 10. There are no page, margin or font size limits for the Price Proposals (Volume II). No fold-out sheets shall be submitted as part of the technical or price submission. The following documents are excluded from the 25-page limit: - One (1) Cover letter for Technical (which includes Technical Approach and Past Performance factors) and Price Proposal - Past Performance information/questionnaires, in accordance with Part 1, Instructions - Price Proposal G. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. H. Questions pertaining to the RFP: All questions must be in writing and submitted by email, no later than 12:00 p.m. on Aug 31, 2011, local time, and emailed to: Ingram.Antoinette@dol.gov. I. The Due Date for Proposals: All proposals must be submitted via email, no later than 12:00p.m. on Sep 9, 2011, local time, to: Ingram.Antoinette@dol.gov. No proposals will be accepted if received after the due date and time set for proposals. The only exception will be as a result of government error. In that case, the due date for proposals will be extended at the Contracting Officer's discretion. J. PART 1 - INSTRUCTIONS TO OFFERORS, CONDITIONS, AND NOTICES TO OFFERORS Technical Approach Factor Within Volume I, the offeror shall demonstrate its understanding of the requirements and provisions of the Performance Work Statement (PWS) and the offeror's technical approach to satisfying the needs of the Government as reflected in the RFP, to include a detailed management plan and with a respective timeline attached. Also, the offeror shall submit resumes of qualified personnel that can provide the technical expertise requirement as specified in the PWS (Note: Resumes of key personnel shall include letters of commitment). Any offer that simply restates or repeats the PWS or makes statements without supporting rationale may be determined to be unacceptable. Past Performance Factor Volume II. Past Performance. Offerors shall submit a description of no more than three prime contracts and sub-contracts performed during the past three (3) years that are relevant to the efforts required by this solicitation. Relevancy is defined as an effort that has a logical connection with the efforts required by this solicitation. Each instance of contractual past performance shall be presented as a contiguous description, but organized into the following sections: (1) Section 1 - Contract Descriptions. The offeror shall include the information contained in Part I A. Contract Identification and Part I B. Identification of Offeror's Representative, of the Past Performance Questionnaire in Volume II. (2) Section 2 - Performance. With respect to the information contained in Section 1, above, offerors shall provide a specific narrative explanation of each contract listed describing the objectives achieved and detailing how the effort is relevant as defined above. For any contracts that did not/do not meet original cost, schedule or technical performance requirements, provide an explanation of the reason(s) for the shortcomings and any corrective actions taken to avoid recurrence. This information shall be contained in Volume II, (3) Section 3 - Past Performance Questionnaire (RFP Attachment 2). For all contracts identified in Section 1, Past Performance Questionnaires must be completed and submitted. The offeror shall complete Part I of the Past Performance Questionnaire and e-mail the questionnaire to both the Contracting Officer for this procurement, and to the Government POC responsible for the past/current contract. The offeror shall instruct the POCs to electronically complete Part II of the Past Performance Questionnaire and e-mail the entire questionnaire to the Contracting Officer no later than the proposal due date, to Antoinette Ingram, email: Ingram.Antoinette@dol.gov. The offeror shall also e-mail to the Contracting Officer a list of all of the POCs who were sent a questionnaire. Price Factor - Volume III. The government will use cost realism in evaluating each offeror's Business/Cost proposal. The government will review the Business/Cost proposal to determine if it is realistic for the work to be performed, reflects a clear understanding of the requirements, and is consistent with the Technical Proposal. A sample Labor Category spreadsheet is attached for usage in submitting your proposal. Note: All evaluation factors other than cost or price when combined, are significantly more important than cost or price. For price evaluation purposes only, the anticipated date of contract award is September 30, 2011. The evaluated contract period of performance is one (1) year, as follows: Base Period: September 30, 2011-Mar 29, 2012 (6 months) Option Period 1: Mar 30, 2012-Sep 29, 2012 (6 months) PART 2 - EVALUATION FACTORS FOR AWARD Basis for Award The Government intends to award a single contract on a best value basis to the offeror that is determined to be the most advantageous to the Government, price and other factors considered. The Government will conduct a best value proposal evaluation. Award will be made to the proposal that is determined to be the most beneficial to the Government using a tradeoff (i.e., best value) process, with appropriate consideration given to the following three evaluation factors: Technical Approach, Past Performance and Price. Offerors are cautioned that award may not necessarily be made to the lowest price offeror. However, if the non-price factors are evaluated as approximately equal between two or more offerors, price may become a determinative factor. The Government may determine that a proposal is unacceptable if the prices proposed are materially unbalanced between line items. Unbalanced pricing exists when, despite an acceptable total evaluated price, the price of one or more contract line items is significantly overstated or understated as indicated by the application of cost or price analysis techniques. A proposal may be rejected if the Contracting Officer determines that the lack of balance poses an unacceptable risk to the Government. Factors and their Relative Importance Relative Order of Importance of Factors. The Technical Approach factor and Performance Risk factor are approximately equal. The Technical Approach factor is more important than the Price factor. The Performance Risk factor is more important than the Price factor. The Technical Approach factor and the Performance Risk factor, when combined, are significantly more important than the Price factor. There are no subfactors. Relative Order of Importance Terminology. The following terminology is provided to give offerors an understanding of the relative order of importance of the evaluation factors. Significantly More Important. The factor is substantially more important than another factor. The factor is given far more consideration than another factor. More Important. The factor is greater in value than another factor, but not as much as a significantly more important factor. The factor is given more consideration than another factor. Comparatively Equal. The factor is nearly the same in value as another factor; any difference is very slight. Technical Approach Factor The technical approach proposal will be evaluated in terms of the contractor's understanding of the requirements, the feasibility of approach, and the completeness of the proposal with respect to meeting the PWS requirements. Specifically, the Government's assessment will include the offeror's ability to manage the contract and the offeror's approach to providing an orderly transition during the transition-in period. Any offer that simply restates or repeats the PWS or makes statements without supporting rationale may be determined to be unacceptable. Past Performance Factor The Government will conduct a performance risk assessment based on the quality, relevancy and currency of the offeror's past performance as it relates to the probability of successful accomplishment of the required effort. The source of information regarding past performance, the context of the data and general trends in performance may also be considered. When assessing performance risk, the Government will focus its inquiry on the past performance of the offeror as it relates to all solicitation requirements. These requirements include all aspects of schedule and performance, including but not limited to, the offeror's record of: (1) quality of services provided; (2) timeliness of performance; and (3) commitment to customer satisfaction. A significant achievement, problem, or lack of relevant data in any element of the work can become an important consideration in the source selection process. A negative finding under any element may result in an overall high risk rating. Therefore, offerors are reminded to include relevant past efforts, including demonstrated corrective actions, in their proposal. Offerors are cautioned that in conducting the performance risk assessment, the Government may use data provided in the offeror's proposal and data obtained from other sources. Since the Government may not necessarily contact all of the sources provided by the offeror, it is the offeror's responsibility to explain the relevance of the data. Offerors are reminded that while the Government may elect to consider data obtained from other sources, the burden of proving good past performance rests with the Offerors. If there is no relevant past performance, the offeror will be evaluated neither favorably nor unfavorably and will be considered an unknown risk. Price Factor Price will be evaluated based upon an assessment of the total proposed price for the base period and the six (6) month option period, for a total of 12 months. Evaluation of options will not obligate the Government to exercise the option(s). The Government will evaluate the total price by summing up the extended amounts of all priced CLINs. The Government will analyze the proposed prices for price reasonableness.
