Description
a.The purpose of this amendment is to extend the closing date to 1 Sept. 2011, 9:00 am EST. b.The measurements of the needed booth have been changed, see PWS for changes. c.A site visit is now a mandatory requirement before submitting a proposal. The site visit will be at Building 1-2739, first floor. No current room numbers as building is not finished. Corner of Hamilton and Randolph, Fort Bragg, N.C between 9:00 am and 11:00 am on Friday the 26th of August. Bring a hard hat this is a construction zone. This is a notice of the Government's intent to solicit, negotiate and award a contract for a Six (6) man AudioMetric Booth. The requirement is for the purchase and installation of a AudioMetric Six Man Booth for the Department of Preventive Medicine, Army Hearing Program located at Womack Army Medical Center (WAMC), Fort Bragg, NC 28310. The proposed requirement and certification of sound proof OSHA requirements. This includes perimeter around the booth and test locations. Booth to be sold and installed by local company (within 100 miles) for proper site coordination and field visits to ensure proper installation. This is also to include proper support post installation should any service be required. Interested persons may identify their interest and capability to respond to the requirement by submiting proposals. Parties interested in submitting a proposal shall make a mandatory site visit at Building 1-2739, first floor. No current room numbers as building is not finished. Parties interested in submitting a proposal shall provide detailed information of their capabilities and certification to clearly meet requirements stated below. 6 man hearing screening booth Requirements: osingle wall oMeets ANSI S3.1-1991 (R1999), "Maximum Permissible Ambient Noise Levels for Audiometric Test Rooms" o120V Light fixtures controlled by external light switches oPrewired to allow operator to control hearing stations from the outside, this includes a minimum of power and connections to operate with the Benson Medical CCA-220 mini and other common audiology testers oCompatible with Benson Medical CCA-200 Mini and calibration simulator oPrefer sliding door model must align with room orientation oMust fit into an area of a room that has low clearances. The area includes 2 beams approximately 8'3" apart one is 7'4" floor to ceiling and the other one is approximately 7'8.5" from floor to ceiling. Overall ceiling height is 8' 5". Distance from overhead obstructions for width is 9'5". A site visit will be required to verify measurements. oUnit must have active air ventilation. oMust meet floor weighting requirements. oDoor must contain a window and door must be at least 2' 9" wide oEach position must be setup with padded stools, and test position divider curtains oColor must be tan/khaki externally oInterior must be carpeted oVibration reduction materials as identified during site visit oParts must be available for install three (3) days after order is placed Installation must be in compliance with all codes, fire protection standards and any other applicable laws.
