Description
Amendment 5 is issued to provide answers to the following questions and provide additional information as follows: 1.QUESTION: a.Duct-work Cleaning-- I was unable to down load technical exhibit for typical layouts and duct work design. Please provide. RESPONSE: Reference Solicitation # W91248-11-T-0027. Refer to Amendment 1, posted 8 August 2011. The file is in.pdf format. b.Replacement of secondary filters-- how many are there and what is the cost of the secondary filter--model of filter-- RESPONSE: Reference Solicitation # W91248-11-T-0027. Refer to Amendment 1, posted 8 August 2011. Refer to the drawings. There are 2 filters located in building 6764 which is typical of all listed buildings. The filters are 14" x 24" metal mesh and are cleanable in accordance with the Statement of Work. If the filters require replacement, the T&M line items (0005 and 0006) will be utilized to replace the filters. Model numbers may vary and shall be field verified; numbers shall be field verified; and prices shall be verified from suppliers. c.Technical exhibit 2 provides duct length (LF) for 28 buildings. Are these the only buildings requiring duct cleaning? RESPONSE: Yes, currently these are the only buildings with systems. d.Does Fort Campbell guarantee that all duct work is clean, all filters clean, all fan and motors clean all vent caps in place all back draft dampers are clean and in place at the time of installation? RESPONSE: No. Reference Solicitation # W91248-11-T-0027. Refer to Amendment 3, posted 12 August 2011: Question #6 - The contractor shall inspect all dryer duct systems at the time of installation and provide a condition report to the Government. 2. QUESTION: Define BOD Date looks like the date building put in service. RESPONSE: Reference Solicitation # W91248-11-T-0027. Amendment 1, posted 8 August 2011, includes the revised bid schedule that supersedes the previously posted bid schedule. The BOD information was deleted and does not apply to this requirement. 3. QUESTION: Please provide the subcontractor that provided the repair service on the washers and dryers and the subcontractor that provided the vent cleaning. RESPONSE: Reference Solicitation # W91248-11-T-0027. Subcontractor information should be sought directly from the current contractor. Refer to Question 1, Amendment 2, posted 12 August 2011 for the current contractor's information. 4. QUESTION: Can preventative maintenance be performed at the same time vent cleaning is performed? RESPONSE: Vent cleaning shall be done IAW the SOW. PM may be done at the same time if it is due. 5. QUESTION: Will Fort Campbell provide a secure warehouse where the new washers and dryers can be stored when delivered at the beginning of the contract? RESPONSE: No, the contractor shall provide storage for the equipment. 6. QUESTION: Can you provide the number of dryer outlets that require three prong and four prong electrical cords? RESPONSE: A count is not available, field verifications is required. Once all machines have been installed the contractor shall provide building number and type of outlet in each building. The contractor shall advise if there are problems with power in each location. 7. QUESTION: Can contractor use flexible vent hose as opposed to rigid metal vent hose? RESPONSE: Yes, the dryer vent hose may be a UL listed connection. 8. QUESTION: Are the 4 front load washers required to be ADA compliant? RESPONSE: Yes. 9. QUESTION: If the operation instructions are on the inside lid of the washer and dryer will that suffice? RESPONSE: Operating instructions on the inside lid is sufficient. 10. QUESTION: How many years has the present lease contract been in effect? RESPONSE: Reference Solicitation # W91248-11-T-0027. Refer to Technical Exhibit 1, contract has been in place for five (5) years. 11. QUESTION: Can you provide the number of service calls per month during the present lease contract? RESPONSE: Reference Solicitation # W91248-11-T-0027. Refer to Technical Exhibit 1, Work Load Data for Service Calls, this information is for the past year. 12. QUESTION: Is the contractor required to purchase and install backdraft dampers on each dryer? RESPONSE: There is no requirement for back draft dampers on dryers. 13. QUESTION: a. The line item schedule calls for 1037 top load washers, 4 front load washers, 431 single dryers, and 406 stack dryers. Technical Exhibit 2: gives the actual number of washers and dryers as: top load washers 863, 442 single dryers and 255 stack dryers. I suspect that the line item totals are the number of units authorized and Technical Exhibit 2 is the actual number of units to install after contract award. Is that correct? So you may increase the number of washers from 863 to 1037 an increase of 174 washers you may decrease the number of single dryers from 442 to 431 or 11 dryers you may increase the number of stack dryers from 255 to 406 or 201 units. My question is my assessment correct? RESPONSE: Reference Solicitation # W91248-11-T-0027. The estimated quantities on both the revised bid schedule and Technical Exhibit 2 are the same. Refer to Amendment 1 posted on 8 August 2011. b. Also if the number of washers and stack dryers is increased over the three year contract will the lease price for the remaining mounts be negotiated? For example if you increase the number of units with 12 months left on the contract the contractor will have only 12 months to amortize the purchase price+ repair cost + loan payment.? RESPONSE: No, the price for additional units will be in accordance with the unit price for the base or option years as shown on the revised bid schedule. Refer to Amendment 1 posted on 8 August 2011. The following Question as posted in amendment 4 on 19 August 2011 is hereby corrected as follows: 2.QUESTION: Is there a formal bid form? RESPONSE: Reference Solicitation # W91248-11-T-0027 and subsequent amendments. Pricing shall be submitted on the bid schedule included in this solicitation. Refer to Amendment 1, posted 8 August 2011, for the revised bid schedule. An additional fax number to send Past Performance Questionnaires to is 270-798-7820. The contact phone number for the POC, Ashley Witmer, identified on the posted Past Performance Questionnaire is corrected to 270-798-7835.
