- Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (i) Solicitation number, Asbestos/Lead-Based Paint Sampling, abatement, and disposal is being issued as a request for quotation (RFQ). (ii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50. (iii) The associated NAICS code is 562910 with a small business size standard of 500 employees. This solicitation is a Total Small Business Set-Aside. (iv) Requested item/description is as follows: One or more Blanket Purchase Agreements shall be established following the below statement of agreement. Please include descriptive literature and/or complete specifications. (v) See attached Statement of Work (10 pages) (vi) Price list at a minimum shall include the sampling of: friable asbestos and lead-based paint containing materials category I nonfriable asbestos and lead-based paint containing materials category II nonfriable asbestos and lead-based paint containing materials category III nonfriable asbestos and lead-based paint containing materials DESCRIPTION OF AGREEMENT FOR ASBESTOS AND LEAD-BASED PAINT SAMPLING, ABATEMENT AND DISPOSAL 1. DUTIES AND RESPONSIBLITIES: a. The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to perform laboratory testing/sampling in addition to other services provided within the price list to be provided by the contractor and approved by the contracting officer. The government will provide the samples to be tested to the contractor. When testing is needed, the 673 CES will contact the contractor and schedule a pickup of the sample needed to be tested. The government will be provided the results of the testing/sampling within 3 days of test completion. b. All services shall be provided within the period specified when a call is placed. c. This agreement will be reviewed and periodically revised to conform to all requirements of statutes, Executive orders, or the FAR and its Supplements. Modifications issued against this BPA or the issuance of a superseding BPA shall be evidence of a revision. d. The contractor shall furnish firm price lists upon expiration of a price list or periodically as required by the contracting officer. 2. EXTENT OF OBLIGATION: a. The Government is obligated only to the extent of authorized calls placed against the blanket purchase agreement (BPA) in accordance with the stated terms and conditions. The Government is responsible for payment of services that have been inspected and accepted by the Government. b. This BPA covers the period of 15 Sept 2011 through 30 September 2016. 3. PURCHASE LIMITATION: No single call issued against this Blanket Purchase Agreement shall exceed $2,500.00. The Contracting Officer is the only individual authorized to place calls in excess of $25,000. 4. INDIVIDUALS AUTHORIZED TO PURCHASE UNDER THE BPA: The only individuals authorized to place calls against the BPA will be appointed by letter by the contracting officer. The vendor shall not accept any calls from any individual not authorized by the Contracting Officer. 5. PRICING a. Prices to the Government shall be as low as, or lower than those charged to the Contractor's most favored customer, in addition to any discounts for prompt payment. b. A current price list must be submitted to and be approved by the Contracting Officer for use on the BPA. The price list shall state the period for which the price list is effective. The minimum period a price list may be in effect is one month. Price lists shall be sent to the 673 CONS/LGCB at fax 552-7496. 6. INVOICES: a. Invoices for services provided under the BPA shall be faxed to 552-9898, 773CES/CEOHS and contain the following information: 1. Name of supplier 2. BPA number 3. Date of service 4. Call number 5. Itemized list of services/materials furnished 6. Quantity, unit price, and extension of each item, less applicable discounts 7. Individual's name that placed the call b. Submit invoices for payment through Government Purchase Card (VISA) c. Invoices shall be submitted monthly when services are rendered. 7. PAYMENT: The method of payment shall be through the Government Purchase Card (VISA) PROVISIONS/CLAUSES: The provision at FAR 52.204-3 ALT 1, Taxpayer Identification applies to this acquisition. The provision at FAR 52.204-7, Central Contractors Registration applies to this acquisition. The provision at FAR 52.204-10, Reporting Executive Compensation and First-Tier Subcontract Awards applies to this acquisition. The provision at FAR 52.209-6, Protecting the Government's Interests When Subcontracting With Contractors Debarred, Suspended, or Proposed for Debarment applies to this acquisition. The provision at FAR 52.212-3, Offeror Representations and Certifications, Commercial Items, applies to this solicitation and the offeror must include a completed copy of this provision with their proposal. FAR 52.212-4, Contract Terms and Conditions Commercial Items, applies to this solicitation. The clause at FAR 52.212-5 DEV, Contract Terms and Conditions Required to Implement Statues or Executive Orders, Commercial Items, applies to this solicitation.
