- Combined Solicitation/Synopsis Ophthalmic Surgery Laser RFP02051100034 General Requirements: The Federal Bureau of Prisons, Federal Medical Center Devens, located at 42 Patton Road, Ayer, MA 01432 is soliciting for the acquisition of an Ophthalmic Surgery Laser. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This acquisition is a total small business set-aside. The NAICS code for this requirement is 334510; the small business size standard is 500 employees. This combined synopsis/solicitation document incorporates clauses and provisions in effect through Federal Acquisition Circular FAC 2005-53. Description of Items: Requested items to be quoted brand name or equal: Item 1: Lumenis SELECTA II, 110V Portable "clip on" 532 nm pulsed laser system. Delivers 0.10 - 2.0mJ of energy, 400 micron spot, 4 degree cone angle. Used with a Haag Streit type slit lamp bio-microscope and attached to the tonometer mount. Plugs into a standard 100V or 220V electrical outlet. Selectra II (tm) includes: footswitch, one pair of safety spectacles, operator manual, laser safety sign. Warranty includes one year all inclusive coverage. Item 2: Lumenis SL 3X 990 Slit Lamp Includes one year warranty Item 3: Lumenis CSO Table Top, Wheelchair Item 4: Lumenis CSO Base, Wheelchair Delivery Schedule: Contractor is responsible for delivery FOB Destination to 36 Independence Drive; Building 1677, Ayer, Massachusetts four weeks after receipt of award. As of January 1, 2005, all potential offerors are required to complete the annual Representations and Certifications Application (ORCA), which should be completed at http://orca.bpn.gov. This information is included in this solicitation in the provision FAR 52.212-1 (June 2008) Instructions to Offerors - Commercial Items. Once you have completed the ORCA requirement, you are no longer required to submit 52.212-3 (May 2011) Offers Representations and Certifications with your quote. Failure to complete either of these registration procedures outlined in this solicitation, may result in elimination from consideration for award. Evaluation Factors: The contract type is a firm fixed price. The Government reserves the right upon award to make formal single or multiple awards. Quotations will be evaluated and award will be based on but not limited to the price, delivery, discounts, warranties, and overall best value to the Government. With your quote provide all specifications. Contractor must be able to deliver items no later than four weeks after receipt of order. The Government will make award to the responsible offeror whose offer conforms to this solicitation and is most advantageous. The proposed price must include all associated charges, fees and delivery cost (FOB Destination).
