- This is a sources sought notice to search for small business vendors that can provide Art Instructor services as described in the attached Statement of Work, dated August 22, 2011. This is not a solicitation and no quotes are being requested at this time. Respond back by Friday, September 9, 2011 to the Contracting Officer and state if you can provide this service along with your business name and point of contact information. STATEMENT OF WORK Art Instructor Revised: August 22, 2011 Description of the work Art instructor will provide therapeutic instruction in fine art through small group sessions or individual sessions in the Community Living Center (CLC) and Hospice. This is a specialized service that utilizes multiple mediums used in fine art. Contractor will demonstrate knowledge in applying age-specific competencies. Type of Services Skilled, knowledgeable artist to assist veterans with functional impairment to express themselves. Art serves as a therapeutic recreation to increase self-esteem and improve mood and fine motor skill. Contract Structure Contractor will provide Eight (8 hours/work per week or 416 hours/year.) Teach 2 art classes per week; each art class is 4 hours. (This includes one hour of set up and clean-up time per class/ with a 3 hour class. Start Date October 1, 2011 thru September 30, 2012 (Contract is for One (1) year). Describe Contractor will: Provide two (2) small group art sessions per week. Sessions to include mask making, mosaics, pastels and charcoal, watercolor and group murals. Enter event captures and complete attendance sheets for each session. Document progress in CPRS. Enter time worked in daily log. Provide quarterly list of needed supplies. Exhibit veteran art work. Adapt art activities to allow/enable every resident to have successful outcomes. Request 30 days in advance any scheduled time off. Comply with Information Security requirements and complete annual training. Recreation Therapist will monitor programs including veteran response and documentation. Lunch, breaks and invoice preparation are not included in contracted hours. Materials to be used: Provided by VA. *NOTE: In the case where hours are not provided as per contract (i.e. contractor illness, needs of residents) unscheduled hours within the allotted contract will be made up on evenings and/or weekends.
