Description
This is a sources sought notice to search for small business vendors that can provide Massage Therapy services as described in the attached Statement of Work, dated August 22, 2011. This is not a solicitation and no quotes are being requested at this time. Please respond back by Friday, September 9, 2011 to the Contracting Officer via e-mail only and state if you can provide this service along with your business name and point of contact information. STATEMENT OF WORK For Community Living Center (CLC) śMassage Therapy ť Revised: August 22, 2011 Purpose of the Contract: Provide Therapeutic massage to CLC residents Description of the work ” Massage Therapist will provide therapeutic chair massage to CLC residents, including Hospice residents at end of life. This is specialized service that requires licensing and specialty area certification. Type of Services ” Skilled professional to provide massage to reduce stress and agitation via chair massage. Activity increases comfort, relaxation and circulation and contributes to overall functional improvements. Contract Structure ” Contractor will provide eight 5.5 hours/week = 286 hours per year. Start Date ” October 1, 2011 through September 30, 2012 (Contract is One (1) year). Describe ” Contractor will: Provide 20 minute chair massages = 5.5 hours/week x 52 weeks = 286 hours/.per year. Be responsible for documenting notes regarding resident contacts and forwarding these to Recreation Therapist. Be responsible for taking attendance each session. Provide education to staff and veteran regarding massage as needed or requested. Request 30 days in advance any scheduled time off. Other duties related to recreation/leisure programming as requested by Recreation Therapist. VA will provide chair and massage lotion. Recreation Therapist will monitor programs including veteran response and documentation. Lunch, breaks and invoice preparation are not included in contracted hours. *NOTE: In the case where hours are not provided as per contract (i.e. contractor illness, needs of residents) unscheduled hours within the allotted contract will be made up on evenings and/or weekends. Contractor must discuss this in advance.
