- 548-12-1-5555-0015 NOTICE OF INTENT/SOURCES SOUGHT & STATEMENT OF WORK The Department of Veterans Affairs Medical Center in West Palm Beach Florida intends on awarding a Sole Source Contract for the following SOW as noted by the justification attached. Market research has shown there is no other suitable vendor, however the intention is to insure through this Sources Sought that the ultimate procurement of the services be made available to any vendor that is indeed capable of providing those services without damage, absence of use and with warranty including ability to perform with the proprietary parameters. Please contact terry.emory@va.gov stating the capability. THIS IS NOT A SOLICITATION, merely market research. I.INTRODUCTION AND SCOPE OF WORK This contract is to provide telephone service support on the DEXIS equipment as identified below, located at the Department of Veterans Affairs Medical Center, West Palm Beach, FL. Services shall be performed by manufacturer trained service engineers. Coverage shall begin on 10/01/11 and end on 9/30/12. Service agreement for DEXIS "DExcare" Xray system to include full phone support and sensor replacement (6). Equipment:DEXIS Dental PACS System DEXIS Sensor SN: 0601022 DEXIS Sensor SN: 0709136 DEXIS Sensor SN: 0834167 DEXIS Sensor SN: 0102331234 DEXIS Sensor SN: 0112132913 DEXIS Sensor SN: 0112134003 II.REQUIREMENTS Contractor shall provide maintenance services in accordance with manufacturer's specifications and as outlined below: 1)Provide all parts, labor, and any updates/upgrades that address efficacy and safety issues. 2)The telephone support shall include 8:00 am to 8:00 pm Monday through Thursday, and 8:00 am 6:00 pm Friday. Provide live remote assistance via modem for hardware/software and technical support, diagnostic callback response. 3)Provide priority overnight replacement of parts including sonsor, PC card, or USB controller. 4)Contractor shall have twenty-four (24) hour access to all needed parts. 5) Contractor shall provide evidence of factory training for each service engineer upon request. 6) Contractor shall apply gummed label, dated and signed, certifying performance and safety to meet manufacturer's published specifications as of that date. III.PROGRESS AND COMPLIANCE A.Vendor shall provide evidence of factory training for each service technician upon request. B. Contractor shall provide certification worksheets on each machine tested. C. Contractor shall contact Biomedical Instrumentation, (561) 422-6658 to arrange for service. D. Service reports shall be provided to Biomedical Engineering within 24 hours of completing service. The report shall include complete details of all work performed. E. The contractor shall provide certification in the form of technical documentation, original invoice, or service documentation that the quality of updates and upgrades is sufficient to guarantee continued performance and operation of the system listed in this contract.
