Solicitation Number: 549-12-1-065-0001 Notice Type: Combined Synopsis/Solicitation Synopsis: T his is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; RFQ ™s are being requested and a written solicitation will not be issued. The solicitation number is 549-12-1-065-0001 and is issued as a Request for Quotes (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. The associated North American Industrial Classification System (NAICS) code for this procurement is 515210 with a small business size standard of $ 15.0 Million. This requirement is a [ Service Disabled Small Business ] set-aside and only qualified offerors may submit quotes. The solicitation pricing will be due on September 7, 2011 @ 12:00:00.0 CST via e-mail to: brian.delaney@va.gov The VHA VISN 17 - 549 “ North Texas requires the following: STATEMENT OF WORK FOR DIRECTV TELEVISION PROGRAMMING AT DALLAS VA MEDICAL CENTER A. GENERAL INFORMATION 1. Title of Project: DIRECTV Television Programming for Dallas VA Medical Center (DVAMC). 2. Scope of Work: The contractor shall provide all labor, supervision and all other resources required to deliver DIRECTV television programming for DVAMC. 3. Background: DIRECTV Television programming is required for 500 cabling drops in this medical center. Televisions exist in inpatient rooms, waiting rooms, conference rooms, and other designated areas. Cabling is in place. 4. Performance Period: Programming will be provided 24 hours a day, seven days a week for one year. Contractor will provide needed repairs during regular government work week, Monday-Friday, 8:00am-4:30pm. 5. Type of Contract: Fixed price will cover costs of DIRECTV programming for all cabling drops (televisions) for entire year (12 months). B. GENERAL REQUIREMENTS Contractor will provide a variety of DIRECTV television programming to include news, sports, weather, and a wide variety of entertainment for hospital television distribution system. Major network (including, but not limited to ABC, NBC, and CBS) affiliated local channels will also be provided. DIRECTV programming will be provided for 500 rooms/drops. C. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES DIRECTV television programming will be provided by the contractor 24 hours a day, seven days a week for the entire hospital. Channels offered will include but are not limited to CNN, FOXNEWS, ESPN, ESPN2, Golf, NFL Network, Weather, AMC, ABC Family, Fox Movie, Lifetime, Lifetime Movie, USA, TNT, TBS, FX, A&E, Discovery Channel, Learning Channel, Animal Planet, National Geographic, History Channel, Science Channel, Food Network, Travel Channel, Nickelodeon, TV Land, SyFy Channel, SPEED TV, Spike TV, Comedy Central, C-SPAN, and S-SPAN II. Also included will be all network affiliated local channels (ABC, CBS, NBC, etc) and other local channels as specified. All associated equipment required for distribution will be serviced by contractor such as receivers, modulators, racks, and similar materials. Contractor will make site visit to Dallas VAMC to assess equipment needs and location variables. Upon any service interruption, contractor will respond to telephone calls and/or emails within four hours and attempt to make repairs within 24 hours during normal business week. Repairs or replacement will be accomplished within 48 hours. Contractor will make all payments to subcontractors before designated due dates. Contractor must avoid any interruption of DIRECTV programming distribution to Dallas VAMC. D. EVALUATED OPTIONAL FEATURES Contractor may offer other optional channels as negotiated with Dallas VAMC if funding is available. E. SCHEDULE FOR DELIVERABLES DIRECTV head-end equipment is already in place at Dallas VAMC. Therefore, DIRECTV programming will be provided without interruption to current service by contractor within one week of award of contract. F. CHANGES TO THE STATEMENT OF WORK Any changes will be negotiated with the Contracting Officer at the Dallas VAMC. G. GOVERNMENT RESPONSIBILITIES Dallas VAMC will provide answers to Contractor questions regarding television distribution system as needed to enable Contractor to deliver appropriate equipment. When service interruption occurs, Dallas VAMC will make telephone call to contractor at first recognition of interruption on first business day of discovery. All details of outage will be given to contractor. Offeror must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. Price quoted must meet or exceed the specifications listed above. The evaluation criteria for this RFQ will be based on the following factor per FAR 15.101-2: Lowest Price Technically Acceptable and must represent the best value for the Government.
